About this file
This document is a procedural guide outlining the steps healthcare providers must follow in the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program. It details a six-step process for providers serving high-risk veterans identified through the program.
The steps include: 1) receiving notification about a high-risk veteran, 2) optionally accessing a dashboard for clinical signals, 3) re-evaluating care through comprehensive record review, 4) considering treatment enhancement strategies like safety planning and increased monitoring, 5) conducting veteran outreach using REACH VET Talking Points, and 6) documenting in CPRS using the national REACH VET PROVIDER NOTE template. Providers must complete all tasks by the fourth Wednesday of each month, with initial veteran outreach required within one week of notification. The document includes both a high-level overview and detailed instructions for each step, emphasizing comprehensive care evaluation, evidence-based treatment, and enhanced communication strategies.
