About this file
This addendum document modifies Centralized Request for Quote (CRFQ) AGR27-07 issued by the State of West Virginia Department of Administration Purchasing Division for debris removal and streambank repair services at Opequon Creek in Berkeley County, West Virginia due to flood damage. The solicitation was issued on August 11, 2026, with bids closing on August 26, 2026 at 1:30 PM. A mandatory pre-bid meeting is scheduled for August 14, 2026 at 11:00 AM, and vendor questions must be submitted by August 19, 2026 at 2:00 PM. Addendum No. 01 attaches required forms including the Construction Bid Submission Review Form (WV-75), Drug Free Workplace Conformance Affidavit (WV-73), Certified Drug-Free Workplace Report Coversheet (WV-72), and Bid Bond Form. No changes were made to the pre-bid meeting date and time, vendor question due date, or bid opening date and time. The work is to be performed for the West Virginia Conservation Agency, with invoicing and delivery to its location in Charleston, West Virginia.
The addendum requires vendors to submit a valid bid bond of at least five percent of the total bid amount, certified by a surety licensed in West Virginia with proper seals and power of attorney documentation. Vendors must acknowledge receipt of all addenda, failure to do so may result in bid disqualification. Additional mandatory requirements include attendance at the pre-bid meeting, execution of a signed bid, compliance with federal and state debarment requirements, valid West Virginia contractor's licensure, a signed drug-free workplace affidavit, and proof of workers' compensation and unemployment compensation compliance. Vendors must also complete a subcontractor listing within one business day of bid opening if applicable. The file does not specify pricing terms, contract duration, or funding sources beyond identifying the West Virginia Conservation Agency as the requesting entity and the State of West Virginia as the obligee.
