This document is a Contract Discrepancy Report (CDR) template used by the Contracting Officer's Representative (COR) to formally document and address contractor performance issues.
The CDR template requires completion of seven key sections: contract number, contractor task manager or on-site representative name, COR name, date and time the discrepancy was observed, detailed description of the discrepancy or problem with any supporting attachments, determination of whether a written corrective action plan is required, and if required, the timeframe for the contractor to submit the plan (specified in days after CDR receipt). The form includes signature lines for both the COR and the contractor's task manager or on-site representative, along with corresponding dates. The COR may initiate a CDR at any time, including when monthly recorded defects for a performance standard exceed allowable limits, when unacceptable performance is determined to be critical in nature and requires formal corrective action, or when an unfavorable trend is detected in contractor performance.
