About this file
This is a Common Carrier Attestation form issued by Pinellas County, Florida for use in conjunction with Request for Proposal 26-0251 for Employee Benefits Concierge & Health Navigation Services. The attestation document requires authorized contractor representatives to confirm compliance with Florida Statutes 92.525 and 908.111, specifically attesting that the contractor will not knowingly transport unauthorized aliens into Florida during the contract term, except to facilitate their detention, removal, or departure. The form requires the contractor's authorized representative to provide their signature, printed name, date, Federal Work Authorization User Identification Number, and the name and number of the associated Pinellas County contract. The contract term is five years effective August 1, 2026, with two optional twelve-month renewal periods, and the County reserves the right to terminate the agreement for cause if the contractor violates the attestation requirements.
This attestation is a mandatory compliance document for all contractors responding to the RFP for employee benefits concierge and health navigation services supporting approximately 3,250 benefits-eligible County employees and their families. Contractors must execute this form along with other required attestations, including the Foreign Countries of Concern Affidavit, Human Trafficking Affidavit, and HIPAA Business Associate Agreement. The RFP also requires contractors to register with the Florida Division of Corporations and comply with E-Verify requirements prior to award. No specific budget ceiling or award value is disclosed in the available documentation, though the County indicates that expenditures will be capped at an amount to be determined following award, with the County reserving the right to unilaterally add or delete locations or services during the contract term.
