HW553 DC006M

This Task Assignment authorizes E&E to conduct monitoring well installation and groundwater monitoring activities in accordance with the March 30, 2020, Scope of Work, Cost Estimate, and Bid Documentation.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Monitoring Well Replacement E&E will install replacement wells MW016R and MW018R. Bids for well installation were solicited from Preferred Drilling Solutions, Ambient Technologies, and Groundwater Protection.

Preferred Drilling Solutions was selected as the lowest bidder. Excess soil will be stored as investigation-derived waste (IDW) on-site in five 55-gallon drums. Bids for soil IDW transport and disposal were solicited from Erwin Remediation, PermaFix, and Clark Environmental. Erwin Remediation was selected as the lowest bidder.

Subtask 3: Annual Groundwater Monitoring E&E will collect groundwater samples from monitoring wells MW001R, MW005, MW006, MW011, MW013, MW016R, MW018R, MW019, MW020, MW032 – MW036, and IW014.

Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). The groundwater sample from MW005 will also be analyzed for color. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum before being air-sparged and disposed of on site.

Subtask 4: Annual Groundwater Monitoring Report E&E will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.

MEMORANDUM

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $19,165.17 with 114.5 labor hours.

E&E may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Re: Monitoring Well Replacement and 2020 Annual Groundwater Monitoring Cost Proposal for the Cinderella Cleaners Site, Lake Park, Palm Beach County, Florida (ERIC_5174, Task Assignment: HW553-DC006M)

Dear Benjamin:

Ecology and Environment, Inc. (E & E) is submitting to the Florida Department of Environmental Protection (FDEP) this monitoring well replacement and annual groundwater monitoring cost proposal package for the above-referenced site.

This cost proposal package includes a monitoring schedule (Table 1), site plan (Figure 1), and a cost estimate form / level of effort. A brief summary of the scope of work is provided below:

Subtask 1: Project Management This subtask consists of project management hours only and is automatically calculated based upon 12.5% of the direct labor costs.

Subtask 2: Monitoring Well Replacement Two monitoring wells (MW-16 and MW-18) will be reinstalled offsite down gradient from the facility. The monitoring well locations are shown on Figure 1. Costs for the installation of an additional monitoring well were requested from Groundwater Protection Inc., Ambient Technologies and Preferred Drilling Solutions. As the lowest bidder, Preferred Drilling Solutions was selected to perform the well installation. Soils generated from well installation activities will be placed in a Florida Department of Transportation approved 55-gallon drums. For costing purposes, it is assumed the soil waste generated will be non-hazardous.

Costs for removal and disposal of IDW drums by Erwin Remediation.

Subtask 3: Annual Groundwater Monitoring During the 2020 annual groundwater monitoring event, groundwater samples will be collected as outlined in Table 1 for laboratory analysis. Well locations are provided on Figure 1. For

QA/QC purposes, one field duplicate and one trip blank will be collected during the field event.

During the collection of groundwater samples, FDEP Standard Operating Procedures will be followed. Additionally, field instrument calibration records (FDEP Form FD9000-8) and groundwater sampling logs (FDEP Form 9000-24) will be completed. Groundwater samples will be collected using a variable speed peristaltic pump and polyethylene tubing. During purging, the polyethylene tubing will be connected to a flow-through cell. A YSI Multiprobe Pro series will be inserted into the flow-through cell via an airtight fitting on the lid. The YSI Multiprobe Pro series, which will be calibrated prior to use, has the capability to measure temperature, pH, dissolved oxygen, conductivity, and oxidation-reduction potential. A calibrated Hach Turbidimeter 2100P will used for the collection of turbidity measurements.

Upon completion of purging of each monitoring well, the flow-through cell will be disconnected and groundwater samples collected from the polyethylene tubing.

Purge water (investigation-derived waste [IDW]) will be containerized in a 55-gallon drum and stored at a secure location onsite. Prior to demobilizing the site, E & E will aerate the containerized purge water for approximately 45 minutes to one hour to reduce potential concentrations of VOCs from the purge water. Once air sparging is complete, E & E will dispose of the treated water on site.

The groundwater samples, one duplicate sample and one trip blank will be shipped to the Advanced Environmental Laboratories, Inc. and analyzed for volatile organic compounds using United States Environmental Protection Agency (USEPA) Method 8260. In addition, the groundwater sample from monitoring well MW005 will be analyzed for the Underground Injection Control parameter color by Standard Method (SM) 2120B.

Depth-to-water will be gauged and recorded in select monitoring wells to determine groundwater elevations across the site and observe seasonal elevation fluctuations.

Subtask 4: Annual Groundwater Monitoring Report

Following receipt of the analytical data for the 2020 annual monitoring event, E & E will prepare a groundwater monitoring letter report. The letter report will include a professional certification, summary of monitoring well installation, tabulations of current and historical field measured parameters and analytical results, a brief discussion of the analytical results, and groundwater contaminant concentration figures, a groundwater elevation map, conclusions, and recommendations.

Upon FDEP approval of the letter report, an electronic copy of the report and updated geospatial information systems data tables will be submitted to FDEP via the FDEP File Transfer Protocol site or by electronic mail.

Schedule The proposed project task schedule for completion of this work is as follows:

Completion of well install:

Completion of groundwater sampling:

Completion of Annual Groundwater Monitoring Report:

Task Assignment End Date:

June 29, 2020 August 17, 2020 October 5, 2020 November 30, 2020
