Task Assignment Change Order Summary
This document is a Task Assignment Notification Change Order Form for Florida Department of Environmental Protection (FDEP) Contract HW553, issued to Ecology & Environment, Inc. (E&E), with Aaron Cohen serving as the DEP Contract Manager and Dan Foss as the Contractor Representative. The change order applies to multiple sites and facility IDs across multiple DEP locations in Tallahassee, Leon County, Florida. The change order, numbered 3, was executed in November 2014 and addresses Investigation Derived Waste (IDW) Management activities required for critical response operations.
Change Order #3 increases the task assignment cost from $6,547.30 to $6,659.83, representing a net increase of $112.53. This increase was previously authorized under Work Change Directive #3 dated October 22, 2014 and was necessitated by the need to change IDW disposal subcontractors at the Coles Cleaners site. The original IDW disposal contractor, PermaFix, was unable to remove two IDW drums rapidly enough to meet FDEP-required timelines and Real Property Owner (RPO) complaints. E&E selected Clark Environmental as the replacement contractor to expedite removal. The final deliverable consists of signed IDW manifests with a due date of January 9, 2015. The invoice schedule includes three milestone payments: $2,500.00 for completion of site activities (subtasks 1-2), $656.27 upon SVE system shutdown completion (subtasks 3-4), and $3,503.56 upon completion of all IDW disposal activities and FDEP approval of signed manifests (subtasks 5-12).
