About this file

Summary

This is a Hazardous Energy Control Policy document issued by the City of Phoenix Water Services Department, effective August 29, 2024, establishing lockout-tagout (LOTO) procedures for maintenance of equipment in which unexpected energization, start-up, or release of stored hazardous energy could harm employees or damage equipment. The policy defines procedures for personal LOTO (protecting one employee requiring no paperwork), group LOTO (protecting multiple employees, trades, or crafts requiring coordination and documentation), and maintenance LOTO (placing equipment out-of-service for the duration of a work group's tasks). The policy applies to all energy sources including electrical, hydraulic, pneumatic, mechanical, gravitational, and chemical hazards, with specific requirements for electrically qualified persons to coordinate electrical hazard LOTO in accordance with NFPA 70E standards. Lockout devices consist of color-coded locks (red for personal and group use, yellow for maintenance use) and standardized tags constructed to withstand environmental conditions. The policy establishes distinct roles for management, project managers, site managers and supervisors, authorized employees, affected employees, and Water Services Department (WSD) Safety staff, each with specific responsibilities for implementing and monitoring LOTO compliance.

The policy requires affected employees to complete LOTO training briefs with annual review, while authorized employees must receive initial training before conducting any LOTO activity with retraining every three years or upon policy revision, job assignment changes, equipment changes, process changes presenting new hazards, or identified deviations. Documentation requirements include centralized maintenance of contractor exchange of information forms, group LOTO forms, absent employee LOTO removal forms, periodic inspection forms, maintenance LOTO forms, and annual audit documentation, all maintained for one year by WSD Safety staff. For contractor-involved LOTO activities on existing equipment, WSD staff coordinates with contractors to ensure energy isolation following maintenance LOTO procedures, with contractors required to submit Maintenance of Plant Operations (MOPO) documentation preferably two weeks prior to activities for WSD Engineering or Water Facilities Supervisor review and approval. The policy mandates periodic inspections of at least one LOTO in progress annually at remote facilities, lift stations, and each plant, with annual comprehensive audits to review inspection findings, evaluate effectiveness of energy control procedures, and document findings for policy updates as needed.

Dept. Record No. S-005

I. PURPOSE

This policy covers the Maintenance of Equipment in which the unexpected energization, start-up, or release of stored hazardous energy could harm employees or damage Equipment. It will define hazardous energy control procedures (LOTO), employee training, and periodic audits meant to fulfill all OSHA requirements.

II. EXCEPTIONS

A. Work on cord and plug connected electric Equipment that is unplugged and under control of the employee performing Maintenance.

B. Water distribution system Hot Tap operations performed on pressurized pipelines, provided:

1. Continuity of service is essential.

2. Shutdown of the system is impractical.

3. Documented procedures are followed.

4. Effective protection is provided for employees.

III. HAZARDOUS ENERGY

A. Electrical Energy

1. Electrically Qualified Person

a. Only Electrically Qualified Persons will coordinate an Electrical Hazard LOTO and establish an Electrically Safe Work Condition.

b. Electrically Qualified Persons will adhere to NFPA 70E and the Water Services

Department (WSD) Electrical Safety standards.

2.2.1 prior to operating any electrical circuit breaker or disconnect for energy isolation or another purpose.

B. Other Hazardous Energy

1. All Energy Sources other than electricity will be locked out using Established Methods and be addressed to include as needed:

a. Hydraulic and pneumatic potential energy: Set the valves in the closed position and lock them into place. Bleed off the energy by opening the pressure relief valves or closing airlines.

b. Mechanical potential energy: Carefully release energy from springs that may still be compressed. If this is not feasible, block the parts that may move if there is a possibility that that spring can transfer energy to it.

c. Gravitational potential energy: Use a safety block or pin to prevent the part of the system that may fall or move

d. Chemical Energy: Locate chemical supply lines to the system and close and lockout the valves. Where possible, bleed lines or cap ends to remove chemicals from the system.
