About this file
This is a Test Hole Report Sheet documenting utility coordination investigations conducted by VacX for the City of Beaverton, Oregon, in connection with pothole repairs and water infrastructure work at the intersection of SW Hart Road and SW Sorrento Road. The document contains four detailed test hole reports (TH #40 through TH #43) completed between July 18 and July 22, 2025, identifying subsurface water utility lines that must be protected during construction. The investigations revealed multiple water distribution pipes including 16-inch cast iron lines (TH #40 and #41), a 12-inch ductile iron line (TH #42), and a 4-inch ductile iron line (TH #43), all owned and operated by the City of Beaverton. Each test hole report includes precise measurements of utility depth, cover depth, horizontal distance from reference points, pavement core thickness, and cross-sectional sketches showing the spatial relationship between utilities and ground surface. All investigations were conducted with reference to a storm crossing point at elevation 216.39 feet, and no utility tell-tales were set during the work.
The test hole investigations provide critical data for the CIP 4211 project involving approximately 1,800 linear feet of zinc-coated ductile iron potable water pipe installation in sizes of 12, 8, 6, and 4 inches along SW Hart Road. Jason Jones of VacX, contactable at (541) 505-5074 extension 105 or (541) 517-0754, prepared all reports and documented utility coordination information available through the Utility Coordination Service at (541) 505-5074. The data establishes baseline conditions for pothole repairs and water infrastructure work in a heavily traveled residential area requiring comprehensive traffic control and flagging operations. No pricing, contract terms, or procurement details are contained in this technical utility report.
