About this file

This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, located in Carson City, Nevada. The form serves as a prerequisite certification for vendors entering into transactions involving Federal funds, contracts, grants, loans, or cooperative agreements with state and local agencies.

The certification requires vendors to attest that no Federal appropriated funds have been paid or will be paid to influence officers or employees of any agency, Members of Congress, or Congressional staff in connection with the awarding of Federal contracts, grants, loans, or cooperative agreements. Vendors must also commit to disclosing any non-Federal funds used for lobbying purposes by submitting Standard Form-LLL, "Disclosure of Lobbying Activities," and to include this certification language in all subaward documents at all tiers, requiring all subrecipients to certify and disclose accordingly. The form requires completion with the date, solicitation number, vendor name, authorized signer name, and authorized signature. Failure to submit the required certification subjects vendors to civil penalties ranging from $10,000 to $100,000 for each failure, as imposed by section 1352 of the U.S. Code. The form was revised in October 2024.
