About this file
This Certification and Contract Offer document pertains to a Juvenile Crime Prevention Community Program contract opportunity in Washington County, Oregon. The program seeks to provide specialized services targeting high-risk youth in middle and alternative schools across Washington County school districts, with anticipated service to approximately 300 youth annually. The program will support full-time and part-time staff positions and provide year-round liaison services, including during summer and vacation periods. Eligible youth will be identified as high-risk through the Oregon Juvenile Crime Prevention Screen and Assessment or referred by the Juvenile Department for early intervention services. Individual youth participation is anticipated to span 3 to 6 months based on individual needs and accountability factors. Services will be offered on a voluntary basis for youth and families not yet involved with the Juvenile Department, though youth referred for delinquency offenses who decline participation will be returned to the Juvenile Department for response. The program expects to serve male and female youth proportionally and individuals from all cultural, ethnic, and racial populations in Washington County.
The certification document requires proposers to certify that neither they nor their principals are presently debarred, suspended, or ineligible from submitting proposals by any federal, state, or local entity. Proposers must also certify that they have not, within three years preceding the offer, been convicted or had civil judgments rendered against them for fraud, criminal offenses related to public contracts, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, tax evasion, or receipt of stolen property. Additionally, proposers must certify they are not presently indicted or criminally or civilly charged with such offenses and have not had contracts terminated for default by any public agency within three years. Proposers unable to certify to any statements must attach an explanation to their offer and must provide immediate written notice to the County if their certification becomes erroneous prior to contract award. The certification states that disclosure of disqualifying items will not necessarily result in award withholding but will be considered in determining proposer responsibility.

2. Have within a three-year period preceding this offer, been convicted or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performance of a public (Federal, state or local) contract or subcontract; violation of Federal or state antitrust statues relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statement, tax evasion, or receiving stolen property;
3. Are presently indicted for, or otherwise criminally or civilly charged by a governmental entity with, commission of any of the offenses enumerated in paragraph 2 of this certification;

The proposer will provide immediate written notice to the County if at any time prior to contract award, the proposer learns that its certification was erroneous when submitted or has become erroneous by reason of changed circumstances.
Where proposer is unable to certify to any of the statements in this certification, or solicitation document, proposer shall attach an explanation to this offer. A certification that any of the items in the above paragraphs exists will not necessarily result in withholding of an award under this solicitation. However, the certification will be considered in connection with a determination of the proposer’s responsibility.
