About this file
This is a Certificate of Partial Payment form designed for use on state and local construction contracts, specifically applicable to Clay County, Florida's Construction of Radar Road project (IFB #25/26-010). The certificate serves as a vendor attestation and payment authorization document for progress payments during the construction of approximately 1,950 linear feet of roadway connecting Town Center Boulevard to US Highway 17. The project encompasses clearing and grubbing, earthwork, traffic control, paving, storm drainage, utility adjustments, landscaping, irrigation, signage, and pavement markings. A mandatory pre-bid meeting is scheduled for October 29, 2025, with bids due November 20, 2025, at 4:00 pm and bid opening on November 21, 2025. The project timeline requires substantial completion within 345 days from Notice to Proceed, with final completion 30 calendar days after punch list delivery.
The Certificate of Partial Payment requires the vendor to certify the accuracy of all items and amounts, confirm full compliance with contract terms and conditions, and attest that all subcontractors and suppliers have received their pro rata shares from previous payments. The form establishes strict payment flow-down requirements, mandating that prime contractors remit payment to subcontractors and suppliers within 10 days of receipt, and that subcontractors similarly pay their sub-tier vendors within 7 days. Bidders must be FDOT pre-qualified in flexible pavement, drainage, grading, sidewalk, pavement marking, roadway signage, and underground utilities; submit a 5% bid bond; self-perform at least 40% of work; and demonstrate a minimum of 5 years in similar business. The project requires performance and payment bonds, comprehensive insurance documentation, and electronic funds transfer capability, with no specific budget disclosed but 107 separate line items indicating substantial project investment.
