About this file
This is a Certificate of Final Payment form associated with Clay County's Lake Geneva Restoration Project in Florida, a Phase I environmental restoration initiative focused on tree removal from approximately 342 acres of exposed lakebed. The project requires contractors to remove trees measuring four inches or greater in diameter, clear vegetation, and prepare the area for future restoration activities while maintaining compliance with environmental regulations and protected species protections. A mandatory pre-bid meeting is scheduled for December 4, 2025, at 10:00 a.m. at Keystone Heights City Hall, with final inquiries due December 9, 2025, at 4:30 p.m. Bids are due December 18, 2025, at 4:30 p.m., with bid opening on December 19, 2025, and an estimated board recommendation date of January 13, 2026. The contract term is twenty-four months from execution with potential extensions for two additional twelve-month periods. Work is restricted to Monday through Friday, 7:30 a.m. to 5:00 p.m., with access limited to four designated points.
The certificate template provided establishes final payment procedures requiring the vendor to certify that all work has been performed in accordance with contract terms and that all subcontractors and suppliers have received their pro rata shares of previous progress payments. The contractor must commit to paying all subcontractors and suppliers their final shares within ten days of receiving the final progress payment. The project is funded in part by a $250,000 Florida Department of Environmental Protection Grant Agreement with no county match requirement. Contractors must provide a five percent bid bond and, if the contract exceeds $200,000, a 100 percent Performance and Payment Bond, with liquidated damages assessed at $100 per day past the final completion date. Required insurance includes Commercial General Liability coverage of $1,000,000 per occurrence and $2,000,000 aggregate. The bid pricing structure includes a Base Bid for tree removal and debris consolidation across all five restoration areas, with two alternate bids covering volunteer group involvement and controlled on-site burning operations.
