About this file
This is a DD Form 1423-1, Contract Data Requirements List (CDRL), specifying a single data item requirement for Contract No. 16251 between the U.S. Air Force (555 SWES / Tinker Air Force Base, Oklahoma) and Fischer Custom Communications (Cage Code 53779). The data item is classified as RELI (Reliability & Maintainability) and applies to the Tinker Pin Tester / RF M880 Amplifier system.
The required data item (A003) is an Accident/Incident Report with the subtitle "Base Document and Notice 1Validation," referenced in the Performance Work Statement (PWS) at paragraph 3.3 and authorized under Data Item Description DI-SAFT-81563. The contractor must submit reports on an as-required (ASREQ) basis, triggered whenever Government Furnished Property (GFP)—specifically the Tinker Pin Tester, Part Number PINS-600, Serial Number 608, or the RF M880 Amplifier—experiences mechanical shock, physical impact, environmental contamination, packaging failure, theft, loss, or damage during any phase of handling or testing. An initial flash notification must be provided within 24 hours of incident discovery via encrypted email or DoD SAFE in searchable PDF format, containing the incident date/time/location, affected component identification, damage narrative, and immediate mitigation steps. A comprehensive final incident assessment report must follow within five business days, including root cause analysis, high-resolution photographs, operational impact assessment, corrective and preventive actions, and disposition recommendations. Deliverables must be transmitted concurrently to the Procuring Contracting Officer, Contracting Officer's Representative (COR/GPOC), and Program Manager, with the contract number, exhibit, CDRL number, and affected system serial number in the email subject line. All documents must display Distribution Statement D, limiting distribution to DoD and U.S. DoD contractors only, with requests for the document referred to 555 SWES, Tinker AFB, Oklahoma. The estimated total price for this CDRL is $0.00, classified under price group NSP (Non-Standard Pricing).
