D. Sustainable Construction Submittals:

2. Low Pollutant-Emitting Materials: show volatile organic compound types and quantities.

E. Certificates: each product complying with specifications and construction with specified performance.

F. Operation and Maintenance Data: start-up, maintenance, troubleshooting, emergency, and shut-down instructions for each operational product

A. Deliver products in manufacturer's original sealed packaging.

B. Legibly mark packaging. Note manufacturer name/brand, type, and manufacture date.

C. Before installation, return/dispose of products within distorted, damaged, or opened packaging.

A. Store products indoors in dry, weathertight facility.

B. Protect products from damage during handling and construction operations.

SECTION 01 00 00

GENERAL REQUIREMENTS

GENERAL

1.1 SAFETY REQUIREMENTS

A. Refer to section 01 35 26 SAFETY REQUIREMENTS for safety and infection control requirements.

1.2 GENERAL INTENTION

A. Contractor shall prepare site for building operations, including demolition/remove existing structures, furnish labor/materials, and perform work for the MRI-Linac project as required by drawings and specifications.

B. Visits to the site by Bidders may be made only by appointment with the Medical Center COR.

C. Offices of HFS Company, 8718 Botts Lane, San Antonio, TX 78217, as Architect-Engineers, will render certain technical services during construction. Such services shall be considered advisory to the

Government and not construed as expressing/implying a contractual act of the Government without affirmations by COR or the duly authorized representative.

D. Contractor/subcontractor employees shall comply with VA security management program and obtain permission of VA police, identified by project/employer restricted from unauthorized access

1.3 STATEMENT OF BID ITEM(S)

A. ITEM I, GENERAL CONSTRUCTION: work includes general construction, alterations, MEP, and IT necessary removal existing construction and certain other items.

1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR

A. Drawings and contract documents may be obtained from the website where the solicitation is posted.

Additional copies will be at Contractor's expense.

1.5 CONSTRUCTION SECURITY REQUIREMENTS

A. Security Plan:

1. Defines physical and administrative security procedures remaining effective for project duration.

2. Contractor is responsible for assuring subcontractors working on the project comply with these regulations.

B. Security Procedures:

1. Contractor’s employees shall not enter project site without an appropriate badge and may be subject to inspection of personal effects when entering/leaving the project site.

2. Before starting work the Contractor shall give 7-days’ notice to the COR so security arrangements can be provided for the employees. This notice is separate from notices required for utility shutdown described later in this section.

3. No photography of VA premises is allowed without written permission of the COR. Patients and staff are not to be photographed at any time.

4. VA reserves the right to close/shut down the project site and order Contractor’s employees off the premises in the event of a national emergency. Contractor may return to the site only with the written approval of the COR.

C. Key Control:

1. Contractor shall provide duplicate keys/lock combinations to the COR for security inspections of each project area including toolboxes/parked machines and take any emergency action.

2. Contractor shall turn over all permanent lock cylinders to VA locksmith for permanent installation. See

Section 08 71 00 DOOR HARDWARE and coordinate.

D. Document Control:

1. Before starting any work, Contractor/subcontractors shall submit an electronic security memorandum describing the approach to following goals and maintain confidentiality of “sensitive information”.

2. Contractor is responsible for safekeeping of all drawings, project manual and other project information which shall be shared only with those with a specific need to accomplish the project.

3. Certain documents, sketches, videos/photographs, and drawings may be marked “Law Enforcement

Sensitive” or “Sensitive Unclassified”. Secure such information in separate containers and limit access to those who need it for the project. Return the information to the COR upon request.

4. Security documents shall not be removed/transmitted from project site without COR’s written approval

5. All paper waste/electronic media (CDs) shall be shredded/destroyed in a manner acceptable to the VA.

6. Notify COR and Site Security Officer when there is a loss or compromise of “sensitive information”.

7. All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document Management Software (EDMS).

Security, access, and maintenance of project drawings (scanned and electronic) shall be performed/tracked through the EDMS system.

“Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.

E. Motor Vehicle Restrictions

1. Vehicle authorization request shall be required for any vehicle entering site. Such request shall be submitted 24 hours before access date & time. Access shall be restricted to picking up/dropping off materials and supplies.

2. No parking by Contractor/subcontractor on Oklahoma City VAMC.

1.6 OPERATIONS AND STORAGE AREAS (FAR 52.236-10)

A. Contractor shall confine all operations (including materials’ storage) on Government premises to areas authorized/approved by the COR. Contractor shall hold/save the Government, officers/agents, free/harmless from liability of any nature occasioned by Contractor's performance.

B. Contractor shall, under regulations prescribed by COR, use only established roadways when and as authorized by the COR. When materials are transported in the work, vehicles shall not be loaded beyond manufacturer’s recommended vehicle capacity or prescribed by Federal law/regulation.

C. Working space and space available for storing materials shall be as determined by the COR.

D. Workers are subject to rules of the Medical Center applicable to their conduct.

E. Execute work in a manner as to minimally interfere with work being done by others. Keep roads clear of construction materials, debris, standing construction equipment and vehicles.

F. Execute work to minimally interfere with normal Medical Center functioning (utility services, fire protection systems, existing equipment, and work being done by others). Use of equipment/tools transmitting vibrations/noises thru the building structure is not permitted in occupied buildings, during construction, except as permitted by COR where required by limited working space.

1. Do not store materials/equipment in other than assigned areas.

2. Schedule delivery of materials/equipment to construction work areas within buildings in quantities sufficient for not more than 2-workdays. Provide unobstructed access to Medical Center areas required to remain in operation.

3. Where access by Medical Center personnel to vacated portions of buildings is not required, storage of

Contractor's materials/equipment will be permitted subject to fire and safety requirements.

G. Utilities Services: where necessary to cut existing pipes, electrical wires, conduits, cables, etc., of utility services, fire protection or communications systems (except telephone), they shall be cut/capped at suitable places shown or as directed by COR. All such actions shall be coordinated with the COR

H. Phasing:

1. Medical Center must maintain its operation 24/7 days/week. Any service interruption must be scheduled/coordinated with the COR ensuring no operational lapses occur. Contractor shall develop a work plan/schedule detailing procedures to be employed, equipment/materials used, interim life safety measure (during the work) and a schedule defining the work’s duration with milestone subtasks.

2. Ensure such executions, Contractor shall furnish COR with a schedule of approximate dates which the

Contractor intends to accomplish work in each specific area. Contractor shall arrange dates to ensure work’s accomplishment mutually agreeable to COR and Contractor

3. Contractor shall take measures/provide material necessary to protect existing equipment/property in construction areas against dust/debris, so equipment/affected areas will not be hindered. Contractor shall permit routes (access or egress) isolated from construction area by temporary partitions and have walking surfaces, lighting, etc. to facilitate patient/staff access. Coordinate alteration work in areas occupied by VA so operations will continue during the construction period.

I. Maintain existing utility services. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, gases, conduits, wires, cables, etc. of utility services or fire protection and communications systems (including telephone), they shall be cut/capped at places as shown or in absence of such indication, as directed by COR.

1. No utility service (water, gas, electricity, or fire protection/communications systems) may be interrupted without prior approval of COR. Electrical work shall be accomplished with all affected circuits/equipment deenergized. When an electrical outage cannot be accomplished, work on any

01 00 00 - 4 energized circuits/equipment shall not commence without a detailed work plan, prior knowledge, and written approval. Refer to Sections 26 05 11 REQUIREMENTS FOR ELECTRICAL

INSTALLATIONS, 27 05 11 REQUIREMENTS FOR COMMUNICATIONS INSTALLATIONS and

28 05 00 COMMON WORK RESULTS FOR ELECTRONIC SAFETY AND SECURITY for additional requirements.

2. Contractor shall submit a request to interrupt any such services to COR, in writing, 7-days in advance of proposed interruption. Request shall state reason, date, exact time of, and approximate duration of such interruption.

3. Contractor will be advised (in writing) of approval, request, or which other date and/or time such interruption will cause least inconvenience to operations. Approved interruption time may occur at other than Contractor's normal working hours.

4. Major interruptions of any system must be requested, in writing, at least 15-calendar days prior to desired time and shall be performed as directed by the COR.

5. In case of a contract construction emergency, service will be interrupted on approval of COR which will be confirmed in writing.

J. Abandoned Lines: all service lines (wires, cables, conduits, ducts, pipes, and hangers/supports), shall be removed back to their source. Those noted to be abandoned but not entirely removed, shall be sealed, capped/plugged at the main, branch or panel they originate from. Lines shall not be capped in finished areas, but removed/sealed, capped, plugged-in ceilings, within furred spaces or walls/partitions, so they are completely behind the finished surfaces.

1.7 ALTERATIONS

A. Before any work started, Contractor shall make a thorough survey with the COR in which alterations occur/areas of anticipated access routes, and furnish a report, signed by both, to the COR. This report shall list by rooms and spaces:

1. Existence/conditions of items (electrical fixtures, equipment, etc.) required by drawings to be reused, relocated, or both.

2. Note any discrepancies between drawings and existing site conditions.

3. Designate areas for working space, materials storage, and access routes to areas where alterations occur/agreed upon by Contractor and COR.

B. Any items required by drawings to be reused, relocated, or both, found during this survey to be nonexistent, or in the COR’s opinion, to be in a condition the use is impossible/impractical, shall be furnished and/or replaced by Contractor with new items per specifications (furnished by Government). Provided the contract work is changed by reason of this subparagraph B, the contract will be modified under provisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.236-2) and "CHANGES" (FAR 52.243-4).

C. 30-days before expected partial/final inspection date, Contractor and COR together shall make a thorough re-survey of involved areas. They shall furnish a report on conditions then existing of flooring, doors, walls, and other surfaces as compared with conditions as noted in first survey report:

1. Re-survey report shall list any damage caused by Contractor to such flooring and other surfaces, despite protection measures and form basis determining extent of repair work required to restore damage caused by Contractor's workers in executing work of this contract.

D. Provide the following measures:

1. Protect interior existing structures from damage and dust. Wherever work performed, floor surfaces to remain in place shall be protected prior to starting work and intact until all work is completed.

1.8 DISPOSAL AND RETENTION

A. Materials and equipment accruing from work removed and demolition shall be disposed of as follows:

1. Reserved items to remain Government property are noted on drawings or in specifications as items to be stored. Items remaining Government property shall be removed in a manner preventing damage detrimental to re-installation/reuse. Store such items as directed by COR.

2. Items not reserved shall become Contractor’s property and removed by Contractor.

3. Portable equipment/furnishing items located in rooms/spaces where work to be done shall remain

Government property. When rooms/spaces vacated by the VA during alteration period, such items not required by drawings/specifications to be relocated/reused will be removed by the Government in advance of work to avoid interfering with Contractor's operation.

1.9 RESTORATION

A. Remove, cut, alter, replace, patch/repair existing work as necessary to install new work. Except as shown/specified, do not cut, alter/remove any structural work, or disturb any ducts, plumbing, gas, or electric work without the COR’s approval. Existing work to be altered/extended and found to be defective, shall be reported to the COR before it is disturbed. Materials and workmanship used in restoring work, shall conform in type and quality to original existing construction, except as otherwise shown or specified.

B. Upon completion of contract, deliver work complete/undamaged. Existing work (walls, ceilings, floors, mechanical and electrical work, etc.) disturbed/removed performing required new work, shall be patched, repaired, reinstalled, and refinished/left in as good condition as existed before commencing work.

C. At Contractor's expense, service/repair damage caused by Contractor's workers to existing piping/conduits, wires, etc. of utility services or fire protection and communications systems (including telephone) not scheduled for discontinuance or abandonment.

D. Expense of repairs to such utilities/systems not shown on drawings/locations which are unknown will be covered by contract time adjustment and price per clause entitled "CHANGES" (FAR 52.243-4) and

"DIFFERING SITE CONDITIONS" (FAR 52.236-2).

1.10 AS-BUILT DRAWINGS

A. Contractor shall maintain 2-full size sets of as-built drawings kept current during construction to include all contract changes, modifications, and clarifications.

B. Each variation shall be shown in same general detail as used in the contract drawings. To ensure compliance, as-built drawings shall be made available for COR review, as often as requested.

C. Contractor shall deliver 2-approved completed sets of as-built drawings in electronic version (scanned

PDF) to the COR within 15-calendar days after COR’s project acceptance

D. Paragraphs A, B, & C shall also apply to all shop drawings.

1.11 WARRANTY MANAGEMENT

A. Develop a warranty management plan containing information per FAR 52.246-21 Warranty of

Construction 30-days before planned pre-warranty conference, submit 2-sets of warranty management plan.

Include required actions/documents to assure the Government receives warranties to which it is entitled.

Plan must be in narrative form containing sufficient detail rendering it suitable for use by future maintenance/repair personnel not necessarily familiar with this contract. Term “status” must include due date and whether item has been submitted or approved. Warranty information made available during construction phase must be submitted to the COR for approval prior to each monthly invoice for payment.

Assemble approved information in a binder. Turn over to the Government upon work’s acceptance.

Construction warranty period begins on date of project acceptance and continue for the product warranty period. A joint 9-month warranty inspection will be conducted, measured from acceptance time, by

Contactor and the COR. Include in the warranty management plan:

1. Roles/responsibilities of personnel associated with warranty process, including points of contact and telephone numbers within Contractor’s company, subcontractors, manufacturers, or suppliers involved.

2. Furnish with each warranty the name, address, and telephone number of guarantor representatives nearest project location.

3. Listing/delivery status of all Warranty Certificates for extended warranty items, to include HVAC balancing, pumps, motors, and commissioned systems such as fire protection and alarm systems, sprinkler systems, etc.

4. List for each warranted equipment item, construction feature or system noting:

a. Item name.

b. Model and serial numbers.

c. Location where installed.

d. Manufacturers and/or suppliers name and phone numbers.

e. Names, addresses and phone numbers of spare part sources.

f. Warranties and warranty terms. Include 1-year overall warranty of construction and construction warranty start date. Extended warranties must be noted with separate warranty expiration dates.

g. Starting point and warranty period duration.

h. Summary of maintenance procedures required to continue the warranty in force.

i. Cross-reference to specific pertinent Operation and Maintenance manuals.

j. Organizations, names, and phone numbers of persons to call for warranty service.

k. Response/repair time expected for warranted equipment.

5. Plans for attendance at 9-month post construction warranty inspections conducted by the Government.

6. Procedure/status tagging of all equipment covered by extended warranties.

7. Copies of instructions posted near selected equipment where operation is critical for warranty and/or safety reasons.

B. Performance & Payment Bonds: remain effective throughout the construction period.

1. In the event Contractor fails to commence/diligently pursue any required construction warranty work, the COR will have the work performed by others. After the work’s completion, the COR will charge remaining construction warranty funds (expenses incurred by the Government while performing the work, including, but not limited to administrative expenses).

2. In the event sufficient funds are not available to cover the construction warranty work performed by the Government at the Contractor’s expense, the COR has the right to recoup expenses from the bonding company.

3. Following written notification of required construction warranty repair work, the Contractor shall respond in a timely manner. Failure to respond will be cause for the COR to proceed against the

Contractor.

C. Pre-Warranty Conference: prior to contract completion, and a time designated by the COR, the Contractor shall meet with the COR to develop a mutual understanding per this section’s requirements.

Communication procedures for notifying Contractor of construction warranty defects, priorities with respect to type of defect, time required for Contractor response, and other details deemed necessary by the

COR for execution of the construction warranty will be established/reviewed at this meeting. In connection with these requirements/time of Contractor’s quality control inspection, furnish name, telephone number, and address of a licensed/bonded company authorized to initiate construction warranty work action on behalf of Contractor. Point of contract will be located within local service area of warranted construction, available/responsive to Government inquiries on warranty work action/status. This requirement doesn’t relieve Contractor of responsibilities in conjunction with other portions of this provision.

D. Following written notification by the COR, the Contractor shall respond to construction warranty service requirements per the “Construction Warranty Service Priority List” and the priority categories listed below.

Submit a report on any repaired warranty item during the warranty period. Include within said report the problem’s cause, date reported, corrective action taken, and when the repair was completed. If Contractor does not perform the construction warranty within the specified timeframe the Government will perform the work and back charge the construction warranty payment item.

1. First-Priority Code 1. Perform onsite inspection to evaluate situation, determine a course of action within 4-hours, initiate work within 6-hours, and work to completion.

2. Second Priority Code 2. Perform onsite inspection to evaluate situation, determine course of action within 8-hours, initiate work within 24-hours and work to completion or relief.

3. Third Priority Code 3. All other work initiated within 3 workdays and work to completion or relief.

4. “Construction Warranty Service Priority List”:

a) Code 1-Life Safety Systems: fire suppression and fire alarm

1.12 TEMPORARY USE OF MECHANICAL AND ELECTRICAL EQUIPMENT

A. Use of new installed mechanical/electrical equipment to provide heat, ventilation, plumbing, light/power permitted subject to written approval and compliance with:

1. Permission to use each unit/system must be given by COR in writing. If equipment not installed or maintained per the written agreement/provisions, COR will withdraw permission for equipment use.

2. Electrical installations used by the equipment shall be completed per drawings/specifications to prevent damage to equipment/electrical systems. Installation of temporary electrical equipment/devices

01 00 00 - 9 per NFPA 70, National Electrical Code Article 590 Temporary Installations. Voltage supplied to each equipment item shall be verified to be correct. Electrical equipment shall be cleaned before use and again before final inspection including vacuum cleaning and wiping clean interior surfaces.

3. Units shall be properly lubricated, balanced, and aligned. Vibrations must be eliminated.

4. Air filtering shall be designed for the system when complete, all filter elements shall be replaced at completion of construction and prior to system testing/balancing

5. All heat production/distribution system components, metering equipment, and condensate returns used in temporary service shall be cleaned/maintained to prevent corrosion during use and inspected prior to acceptance by the Government.

B. Prior to final inspection, the equipment or parts used which show wear and tear beyond normal, shall be replaced at no additional cost to the Government.

C. Any damage to the equipment or excessive wear due to prolonged use will be repaired/replaced by the

Contractor at their expense.

1.13 TEMPORARY TOILETS

A. Contractor may (for Contractor's workers) use toilet accommodations as assigned by COR. Contractor shall keep such places clean and responsible for any damage done by Contractor's workers. Failure to maintain satisfactory condition in toilets will deprive Contractor of the privilege to use such toilets.

1.14 AVAILABILITY AND USE OF UTILITY SERVICES

A. Government shall make required amounts of utilities available to Contractor from existing outlets and supplies, as specified in the contract. Contractor shall conserve any utilities furnished without charge.

B. Electricity (Construction and Testing): furnish all temporary electric services.

1. Obtain electricity by connecting to electrical distribution system available at no cost to the Contractor.

1.15 NEW TELEPHONE EQUIPMENT

A. Contractor shall coordinate with installation work of telephone equipment by others. This work shall be completed before the building is turned over to VA.

1.16 TESTS

A. Per Section 23 05 93 Contractor shall provide a written testing/commissioning plan complete with component level, equipment level, sub-system level and system level breakdowns. Plan will provide a schedule and written sequence of what will be tested and what the expected outcome will be. Prior to commencing work this document will be submitted for approval. Contractor shall document the results of approved plan/submit for approval with the as built documentation.

B. Pre-test mechanical/electrical equipment/systems making required corrections for proper operation of such systems before requesting final tests. Final test will not be conducted unless pre-tested.

C. Conduct final tests required in various specification sections in presence of an authorized COR representative. Contractor shall furnish labor, materials, equipment, instruments, and forms to conduct & record such tests.

D. Mechanical/electrical systems shall be balanced, controlled, and coordinated. Entire system must be coordinated to work together during normal operation to produce results for which the system is designed.

For example, air conditioning supply air is one part of entire system providing comfort conditions. Other related components are return air, exhaust air, electricity, etc.

E. All related components as defined above shall be functioning when any system component is tested. Tests shall be completed within a time during which operating and environmental conditions remain constant typical of the design conditions.

F. Individual test result of any component, where required, will be accepted when submitted with test results of related components and of the entire system.

1.17 INSTRUCTIONS

A. Contractor shall furnish Maintenance and Operating manuals (hard copies and electronic) when required by the various specification sections and as hereinafter specified.

B. Maintenance/operating manuals. 1-compact disc, 3-hard copies, and 1-electronic copy for each separate piece of equipment delivered to the COR coincidental with equipment delivery to job site. Manuals shall be complete, detailed guides for maintenance and operation of equipment as well as include complete information necessary for starting and adjusting. Manuals shall include an index covering component parts cross-referenced to diagrams and illustrations. The function of equipment, component, control shall be clearly/thoroughly explained. All necessary precautions for the equipment’s operation and reason for each precaution shall be clearly set forth. Manuals must reference exact model, style/size of the equipment and system being furnished. Manuals referencing equipment a different model, style, and size than that furnished will not be accepted.

C. Contractor shall provide qualified, factory-trained manufacturer representatives to give detailed training to assigned VA personnel in the operation/complete maintenance for each piece of equipment. All training will be at job site. These requirements are detailed in the various technical sections. Instructors for component equipment in a system shall be available until instructions for included system items have been completed. All instruction periods to be scheduled by the COR and considered concluded when the COR is satisfied regarding coverage. Contractor shall submit a course outline with associated material to the COR for review/approval prior to scheduling training ensuring subject matter covers contractual requirements.

VA reserves the right to request removal of/substitution for an instructor who does not demonstrate sufficient qualifications per the requirements for instructors above.

1.18 GOVERNMENT-FURNISHED PROPERTY

A. The Government shall deliver to Contractor, Government-furnished property shown on the drawings.

B. Equipment furnished by Government to be installed by Contractor will be furnished at the Medical Center.

C. Contractor shall be prepared to receive equipment from Government and store/place minimum 90-days before project’s Completion Date

1. Equipment storage space will be provided by the Government. Contractor shall unload/store such equipment therein upon its receipt at the Medical Center.

D. Notify COR in writing, 60-days in advance, when Contractor is prepared to receive Government furnished equipment. Arrangements will be made by the Government for equipment delivery.

1. Upon equipment delivery, Contractor shall arrange for a joint inspection with a Government representative. At such time Contractor shall acknowledge receipt of equipment described, make notations, and furnish a written statement as to its condition or shortages.

2. Contractor thereafter is responsible for equipment until such time as acceptance of contract work is made by the Government.

E. Equipment furnished by the Government will be delivered in a partially assembled condition per standard commercial practices, complete fittings, fastenings, and appliances necessary for connections to respective services installed under contract. All fittings/appliances (couplings, conduits, cables, etc.) necessary to make the connection between Government furnished equipment and utility stub-up furnished/installed by

Contractor at no additional cost to the Government.

F. Assemble/install the Government furnished equipment in place ready for proper operation per specifications and drawings.

G. Furnish supervision of equipment installation at project site by qualified factory trained technicians regularly employed by the equipment manufacturer.

1.19 RELOCATED EQUIPMENT ITEMS

A. Perform equipment or item relocation at such times and in such a manner as directed by the COR.

B. Provide all mechanical/electrical service connections, fittings, fastenings, other materials necessary for assembly/installation of relocated equipment. Leave such equipment in proper operating condition.

C. All service lines as noted above for relocated equipment shall be in place at point of relocation ready for use before any existing equipment is disconnected. Make relocated existing equipment ready for operation or use after reinstallation.

SECTION 01 00 11

INFECTION CONTROL MEASURES

1.0 GENERAL INTENTION: this document pertains to station policy for construction projects performed at the Oklahoma VA Medical Center in Oklahoma City, OK. Both staff and Contractor shall adhere to regulations and requirements. Review this information as orientation with personnel performing work on site. Where the requirements as outlined in this and Section 01 00 00 GENERAL REQUIREMENTS are differing, the more stringent shall apply.

2.0 REQUIREMENTS:

A. Security:

1. Secure construction areas, especially against entry of unauthorized individuals including patients.

2. Notify COR for permission to work after hours and weekends. Standard work hours will be Monday through Friday. Time of day will be determined by scope of work to be performed and COR approval.

B. Key Security:

1. A limited number of keys will be issued to the Contractor.

2. Reference Section 01 00 00 GENERAL REQUIREMENTS for procedures in case Contractor loses a key.

3. Ensure all doors leading to/from construction are either monitored or locked preventing access to by unauthorized persons.

C. General Safety:

1. Follow Federal safety/health regulations.

2. Maintain safety in construction site/area per contract provisions including OSHA Regulations, National Fire Protection Association (NFPA) 70, National Electric Code, and NFPA 101 Life Safety Code.

3. Provide Personal Protective Equipment (PPE) for employees.

4. Post appropriate signs in specific hazardous areas.

D. Safety Inspections: OSHA will perform safety inspections of contract operations. Written reports of unsafe practices/conditions will be reported to COR for attention and resolution.

E. Airborne Dust Control During Construction:

1. Dust generation is a major concern within staff and patient occupied buildings. All efforts will be directed reducing airborne generated dust to lowest level feasible. This may be accomplished by numerous methods which include misting the area with water or use of tools attached to high efficiency particulate air (HEPA) filtering vacuums. Where large amounts of materials may be disturbed establishment of full ceiling-to-floor plastic barriers may be required.

2. Classification of Jobs: see ICRA (Attachment A)

H. Infection Control Procedures: see ICRA (Attachment A)

1. Implement requirements of VAMC Infection Control Risk Assessment (ICRA). ICRA may monitor dust in construction work area and require Contractor to take corrective action if safe levels are exceeded.

2. Establish/maintain a dust control program as part of Contractor’s infection preventive measures per ICRA Group guidelines. Prior to start of work, prepare a plan detailing project-specific dust protection measures, including periodic status reports, and submit to COR for compliance review with contract requirements per Section 01 33 23 SHOP DRAWINGS, PRODUCT DATA, AND SAMPLES. All personnel involved in

01 00 11 - 2 construction/renovation activity shall be educated/trained in infection prevention measures established by the Medical Center.

3. The following preventive measure shall be adopted during construction to keep down dust and prevent mold.

Dampen debris to keep down dust.

I. Environmental Protection:

1. Contractor shall be aware that environmental protection requirements of each contract are regarded.

Adherence to the requirements is subject to continuing scrutiny and backed by penalties. These environmental requirements will be strictly enforced.

2. No hazardous materials will be disposed of on Government property. All waste will be hauled off-site or disposed in Contractor owned and operated waste removal containers.

3. A copy of all waste manifests for special or hazardous wastes will be forwarded to COR. Environmental requirements will be strictly enforced.

J. Permit Required Confined Spaces:

1. Contractor performing work will follow OSHA Standard requirements for working in confined spaces.

2. Confined spaces: areas large enough to be entered, limited egress/exit potential, and not designed for permanent human occupancy. If a space meeting this definition is encountered and is a suspected confined space, contact the COR.

3. Contractor performing work in confined spaces is responsible for complying with all applicable standards and regulations.

K. Housekeeping:

1. Make every effort to keep dust/noise to a minimum. Take precautions to protect VA equipment from damage including excessive dust.

2. Maintain clear access to mechanical, electrical devices, equipment, and main corridors. This ensures access to existing systems in the event of an emergency.

3. Clean area of all construction debris and dust upon completion of demolition and/or renovation.

L. Hot Work Permits:

1. Any hot work operations including cutting, welding, brazing, soldering, grinding, or any other similar activity, will require a Hot Work Permit obtained by the Contractor from COR (Hot Work Permit shown in Attachment C). Contractor will be responsible to conform with Medical Center regulations, policies and procedures concerning Hot Work Permits as outlined:

a. Prior to the hot work performance, a request for Hot Work Permit will be made to COR. Submit request at least 48 hours in advance of work beginning. Designate Contractor responsible project-site fire prevention program manager to supervise permitted hot work.

b. COR will inspect the area ensuring the requirements of NFPA 241, and OSHA standards have been satisfied. Hot Work Permit will be granted and posted in the immediate work area.

c. Hot Work Permit will apply to the location identified on the permit. If additional areas involve hot work, additional permits must be requested.

d. Upon hot work completion, COR will be notified by Contractor to perform a re-inspection of the area.

2. Do not use any Medical Center extinguishers for standby purpose while conducting hot work. Contractor is required to supply Class ABC extinguishers. Medical Center extinguishers only to be used in a fire event

M. Emergency Medical Services: stabilization purposes available for Contractors, dial 911 when inside the building. Report emergency’s nature and location. Operator will coordinate outside emergency assistance based on the emergency’s nature.

N. Use of Government-Owned Material and Equipment is prohibited.

O. Superintendent Communications: during this contract’s performance Contractor’s Superintendent is to be available by cellular phone. At contract’s beginning/prior to beginning any construction, supply COR with the Superintendent’s telephone number.

P. Contractor parking shall be as referenced in specification section 01 00 00 GENERAL REQUIREMENTS. No subcontractor parking will be permitted.

Q. Contractor's trailers: as referenced in specification section 01 00 00 GENERAL REQUIREMENTS.

R. No smoking is permitted on VA property. Any smoking on VA property is subject to a fine without warning.

S. Fluorescent (PCB Containing) Fixtures: all fixtures being removed shall have ballasts removed and turned over to VAMC Safety Officer for disposal. All other fixture components to be disposed of by the Contractor.

T. Interim Life Safety: Contractor shall coordinate, evaluate/institute Interim Life Safety Measures, with the COR, to temporarily compensate for hazards posed by life safety deficiencies. (Attachment D)

U. COVID-19: Masking and Social Distancing required, while indoors.
