Pre-Proposal Site Visit Meeting Agenda RFQ 36C259-23-R-0031 / Project #635-CSI-022 MRI Linac

The contractor is to provide all labor, tools, materials, equipment, and supervision required to provide the services MRI Linac at OKC VA Health Care System Campus.

Introductions:

Denver Robb, CS Joe Wingfield, CO Ryan Guthrie, COR/Cody Cooper, Project Manager

QUESTIONS During site visit MUST BE WRITTEN DOWN

Important Dates:

· Questions are due on February 27, 2023 at 10:00 AM MT prior to closing of RFQ, any questions submitted after this time will not be reviewed or answered! Submit to Denver.Robb@va.gov

· We’ll get the answers back to you in a timely manner

· Quotes are due on March 10, 2023, at 2:00 PM MT via email

· Prospective offerors are prohibited from attempting to gain information by inquiring with VA employees about project other than through written questions submitted to Contracting Officer.

· Prospective offerors are prohibited from visiting site other than through formal invitation by Contracting Officer.

· Attendees of walk-through are to stay as a group while walking to various locations and are to leave the future project site once the walk-through is concluded.

· All VA campuses are Tobacco Free, including but not limited to, smokeless tobacco and vaping.

· All Federal and local COVID requirements shall be followed for the duration of this project.

· Confirmation of construction range is: $2,000,000 - $5,000,000.

· Bid Guarantee & Payment/Performance Bonds: Bid Guarantee, Payment & Performance Bonds are required prior to commencing any work.

· Selected Contractor shall begin performance within 10 days after Notice To Proceed (NTP) and complete within 270 days.

· 852.219-75 VA Notice of Limitations on Subcontracting-Certificate of Compliance for Services and Construction. Ensure you submit with quote. Also ensure OSHA and EMR Rating are submitted with quote.
