Building state workplace health program expertise and capacity using CDC tools and resources

Jason Lang bzl0@cdc.gov

770-488-5597

Public health, health promotion, and/or wellness professionals working in chronic disease prevention programs within state and local health departments.

$300,000

Purpose: Increasing health care costs and health-related decreases in worker productivity are leading American businesses to examine strategies to improve employee health and contain health costs that are largely driven by chronic diseases and related lifestyle choices. Employers are recognizing the role they can play in creating a healthy work environment and providing their employees with opportunities for healthy lifestyle choices. Workplace health programs not only benefit individual employees but make good business sense.

The approach that has proven most effective is to implement an evidence-based comprehensive health promotion program that includes individual risk reduction programs, coupled with environmental supports for healthy behaviors and is coordinated and integrated with other wellness activities. However, only 6.9% of employers offer a comprehensive worksite health promotion program, according to a 2004 national survey. Several studies have concluded that well designed worksite health promotion programs can improve the health of employees and save money for employers instituting these programs.

The goal of this effort is utilize CDC workplace health tools and resources to compliment, amplify, and/or expand state-based efforts to build effective, comprehensive evidence-based workplace health programs in private and public employers in states.

Proposed Activities:

Using CDC’s workplace health model for program development (https://www.cdc.gov/workplacehealthpromotion/model/index.html), the recipient will engage and recruit employers in multiple states to build or expand evidence-based workplace health programs offered to employees with the expected outcome of improved worker health and well-being over time. The CDC Workplace Health

Model emphasizes four main steps:

Step 1 – Workplace Health Assessment

An assessment should aim to capture a picture of the many factors that influence employee health including: individual level factors such as lifestyle choices, the work environment (e.g, physical working conditions and social support), and the organizational level (e.g., culture, policies, and practices). Employers will use the CDC Workplace Health ScoreCard

State-based and local governmental public health agencies will develop knowledge and skill related to engaging public and private employers in workplace-based health promotion and disease prevention intervention strategies. Many of these interventions have been translated from other settings such as schools and the broader community offering opportunities to create synergy and leverage existing capacity. Partnership with private sector entities interested in health will be created broading the reach of the public health agency and creating additional channels to disseminate information. Staff will be exposed to a number of existing tools and resources to support employers in their states which can be leveraged to achieve state goals without a significant investment in infrastructure and resource development allowing for quicker implementation and additional investment in related activities.

The budget should adhere to OSTLTS and OFR requirements per the funding announcement. The organization should provide a qualified individual dedicated to lead this project. Cost will include staffing support, printing, mailing or other trainee and participant outreach, webinar hosting, evaluation data collection and analysis, training logistic support, travel.

11 CBA Program Outcomes (Check one or more):*

Short-Term

The purpose of the Walkability Action Institute (WAI) Project is to create, support, and evaluate cross-disciplinary teams from across the country in geographic areas represented by metropolitan and other regional planning organizations to implement policy, system, and environmental supports for environments supporting walking and other forms of active living. The project should reinforce significant national public health policy statements supporting walking, including the Surgeon General’s Call to Action to Promote

Walking and Walkable Communities and the National Physical Activity Guidelines.

Newly formed teams should represent cross-disciplinary fields with an interest in promoting policy, system, and environmental supports for environments supporting walking and other forms of active living. Each team must have 4-6 members, including at minimum: one public health professional, one transportation engineering or planning professional, and one elected decision maker with the authority and expertise to support the purpose of the project. Other members may be made up from the fields of economic development, planning, parks and recreation, among others, but each member must have the authority and expertise to support the purpose of the project.

This year’s project has two parts. Part one involves creating a plan for evaluating key success elements from Walkability Institutes held in previous years and developing strategies to scale up the scope and reach of the Institute in future years. Part two involves recruiting another cohort of teams, as described above, to participate in a 2019 Walkability Action Institute.

Part 1. This project element involves creating a plan to scale up the scope and reach of the Walkability Action Institute concept. It must build upon the experiences of the previous four rounds of Institutes to:

• Identify all of the primary and secondary positive impacts of the previous Institutes, including elements such as, but not limited to:

o the individual members of multiple workforces trained in walkable community best practices o the creation of a proven and replicable curriculum o the creation of a cadre of faculty who have experience teaching within an Institute o the number of policies (complete streets, master plans, etc.), systems (safe routes to school, transit improvements, etc.) and environmental improvements (sidewalks, bike lanes, traffic calming, etc.) created or implemented.

• Document the critical elements that led to these positive impacts

• Create a plan to expand the scale and scope of the WAI concept to efficiently and effectively reach more individuals and to cover more of the country without losing the quality of outcome and impact previously demonstrated.

Part 2. This project element will build on the success of the previous WAI cohorts by bringing together at least 8 new teams

(described above) from geographic areas representing metropolitan or other regional planning organizations. The applicant will be responsible for soliciting applications and managing a competitive selction process, as well as be responsible for planning, implementing, and evaluating a minimum of a three day training for teams of 4-6 people. The applicant will provide travel and per diem for the team participants. All teams should meet the criteria listed above as well as include at least one person representing the area’s Metropolitan Planning Organization or other similar Regional Planning Organization that has the authority and expertise to support the purpose of the project. Each team will be expected to develop action plans no more than 30 days after the completion of the training. These action plans must include specific elements related to integrating policy, systems, and environmental supports into the regional planning process of the jurisdiction. The training will include faculty/experts from the field of walking and walkability, transportation engineering, active transportation planning, active living environments, integrating health into metropolitan planning, metropolitan planning professional organizations and possibly other active living fields including bicycling, public transit and smart growth communities.

The project will support the completion of action plans from all the teams, facilitate ongoing communication between the teams, organize peer learning opportunities, monitor progress on implementing action plans, and the development of success stories resulting from the project. In addition, the project may include in person technical assistance or follow-up in person sessions as resources allow as well as the creation of communities of practice created from all of the action teams trained. At the end of the project period, the project should yield action plans from the teams, a short report on the progress teams have made accomplishing their plans, a report on a small set of evaluation measures, and at least one success story resulting from the project.

Public health professionals working in chronic disease prevention programs within state, local and other health department types

$250,000

The Centers for Disease Control and Prevention (CDC) Alzheimer’s Disease and Healthy Aging Program defines “Healthy aging” as the development and maintenance of physical, mental, and social well-being and function in older adults. Healthy aging is most likely to be achieved in physical environments and communities that are safe and support adopting and maintaining attitudes and behaviors known to promote health and well-being, as well as through the effective use of health services and community programs to prevent or minimize the impacts of acute and chronic disease, including Alzheimer’s Disease and other dementias.

The Alzheimer’s Association and the Centers for Disease Control and Prevention (CDC) have developed the third in a series of road maps to advance cognitive health as an integral component of public health. The Healthy Brain Initiative’s State and Local Public

Health Partnerships to Address Dementia, The 2018-2023 Road Map), outlines how state and local public health agencies and their partners can continue to promote cognitive health, address cognitive impairment for people living in the community, and help meet the needs of caregivers.

The capacity for community care providers to provide adequate care and coordination of servives in tandem with health care providers varies widely within each state and across the nation. This project is intended to create a Road Map specifically for

Caregiving aimed at providing suggested actions that would be most realistic and impactful in increasing the ability of State and Local health Departments to address this need.

CDC plans to work with a leading national organization to develop Caregiving Road Map for State and Local health departments and other supplemental materials based on The Healthy Brain Initiative’s State and Local Public Health Partnerships to Address Dementia, The 2018-2023 Road Map) – to build capacity and support state and local governments and their partners to positively impact caregivers and their families (physical, health promotion for patient & caregivers, social, financial). Additionally, analytic support will secured for Healthy People 2020 analyses.

Purpose: The US has experienced tremendous success in reducing tobacco use over the past half century. For some populations, being exposed to tobacco use or second-hand smoke is a rare occurrence. However, for other populations this exposure is part of their daily life. There continues to be a lack of understanding of tobacco-related health disparities in the US.

In 2017/18, CDC’s Office on Smoking and Health (OSH) had a cooperative agreement with ChangeLab Solutions to determine effective messages to increase public understanding of tobacco-related health disparities. ChangeLab Solutions contracted with Frameworks to develop a framing strategy, including message development and testing. An expert panel was established to inform a high-level summary of tobacco-related health disparities, which was supplemented by a literature search. Frameworks used this summary in combination with cultural models drawn from anthropology to identify gaps in understanding of these disparities between public health experts and the general population. Once these gaps were identified, message frames to increase public understanding of the disparities were be developed for pre-testing with the general population.

Both qualitative and quantitative data were analyzed to develop a summary of findings to include expressed values of the target audience, influential messengers/sources of information, and final message frame recommendations.

The next phase of the project would focus on implementation of the message frame and recommendations for communicating tobacco-related disparities in the 21st century. The grantee would develop a strategic implementation plan based on the final message frame recommendations. The implementation plan should include specific strategies for CDC to incorporate the message frames into its own tobacco control efforts, and strategies to disseminate and train its grantees, partners and other stakeholders on how to incorporate it into their work.

The grantee would convene stakeholders to discuss how best to implement the final message frame recommendations to key tobacco stakeholders, including national partners, and state and local health departments.

Proposed Activities:

Convene stakeholders to discuss implementation strategies

Develop a final strategic implementation plan

Disseminate and train grantees, partners on how to incorporate the message frame recommendations

Expected Outputs:

Develop a strategic plan including strategies for implementing message frame recommendations

Expected Outcomes:

Guide tobacco control efforts needed to reach underserved populations
