About this file
This is a Safety Data Sheet template that offerors must complete as part of their proposal submission for Naval Undersea Warfare Center (NUWC) Division, Keyport's Building 5945 Electrical Upgrade solicitation (N0025326Q0010). The form requires offerors to provide comprehensive safety performance data and management information for evaluation as a scoring factor in the RFQ. Offerors must submit five years of historical safety data (2021–2025) including OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) rates and Total Recordable Case (TRC) rates, reported as decimal figures. For any missing years, offerors must affirmatively explain the absence of data. Any negative trends above moderate risk thresholds (DART rates of 3.0 or greater, or TCR rates of 4.5 or greater) require detailed explanations with documentation of corrective actions taken for improvement.
The narrative portion of the Safety Data Sheet, limited to two pages, requires offerors to describe their Safety Management System (SMS) implementation and execution, including management and employee involvement, hazard prevention and control, worksite analysis, and safety training standards used for benchmarking. Offerors must also detail their process for evaluating and selecting subcontractors, as well as their oversight procedures for ensuring subcontractor safety compliance throughout contract performance. For Joint Ventures or Partnerships, each contractor partner must provide a separate Safety Data Sheet, though only one consolidated safety narrative is required.
