Guyed Tower Maintenance and Condition Assessment Report
This document is a comprehensive maintenance and condition assessment report for a guyed tower located in Maynard, MA, conducted following the ANSI/TIA-222-G standard. The inspection evaluated the tower's structural integrity across multiple categories including structure condition, finish, lighting systems, grounding, antennas and lines, appurtenances, insulators, guys, concrete foundations, guyed mast anchors, and tower alignment.
The assessment identified several deficiencies requiring repair. Critical findings include: loose and missing bolts on guy pull-offs requiring replacement; rust and corrosion on B-leg flange at 70-ft and rigging block at 125-ft requiring steel brush cleaning and galvanizing compound application; climbing facilities lacking safety climb installation; inadequate guy tensions on multiple paths (Path A guy 1, Path B guys 1-2, and Path C guys 1 and 3R measured below acceptable tolerance ranges); disconnected grounding wire on Gamma guy requiring reconnection; loose lightning rod connection requiring tightening and installation of grounding wires; inadequate stabilizer arm and connection on antenna feed line; and surface corrosion on Beta anchor requiring treatment. Guy tension measurements detailed in Appendix A indicate that of the 11 measurable guy locations, four measured as "LOW" and one path's tensions could not be assessed due to ground-level insulators requiring crane assistance. Tower plumb and twist measurements in Appendix B confirm the structure remains within ANSI/TIA-222-G-2005 recommended limits for alignment and deflection across all reference elevations from 0-ft to 190-ft above concrete.
