Brand Name Justification (SSJ) for Requirements under the Simplified Acquisition Threshold
1. Identification of the Agency and Contracting Activity: This justification is executed by the Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division – West (HCD-W), to meet the requirements of the Madigan Army Medical Center, 9040A Jackson Ave, Joint Base Lewis- McChord, WA 98431.

Description of Services or Supplies and Estimated Value: Madigan Army Medical Center, JBLM request the purchase of IDEMIA-PIV-100 media reader and FIPSlink V-PIV Visitors Cards system for Security of the Medical Treatment Facility (MTF). The Awardee SHALL provide all required equipment and supplies.

4. Authority: Procurement is conducted under Federal Acquisition Regulations (FAR) Subpart 13.5, in accordance with 41 U.S.C. 1901, Simplified Acquisition Procedures for commercial items.
☒ FAR 13.106-1(b)(1): For purchases not exceeding the simplified acquisition threshold, only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name, or industrial mobilization).
5. Reason for Authority Cited: The objective of this Brand Name award is to procure SSI 3085 Battery-Powered 120V Table and HarmonyAIR ALYON Dual (and Single) Lights, Non-Camera Ready for use at Naval Hospital-Guam. have a requirement for an operating table and surgical lights to perform clinical procedures in Urology clinic. The purpose of this requirement is to provide replacement of aging equipment which was acquired in July 2006 with a 15-year life expectancy. These units are of one of one and used for all clinical procedures conducted in the clinic. Without these units, clinic procedures would not be possible, thus losing beneficiaries to out-of-town deferment, and delaying patient care.
6. Market Research: Market research was conducted August 2025 via mandatory and suggested sources (AbilityOne, UNICOR, SBA Quick Search, MMC/ECAT, GSA) which returned negative results. The existing surgical table and light have reached the end of their service life and are no longer supportable.
These units are used for all clinical procedures conducted in the clinic. Without these units, clinic procedures would not be possible, thus losing beneficiaries to out-of-town deferment, and delaying patient care. New equipment will provide advanced features, increased reliability, and compliance with current clinical and safety standards. Internet searches conducted determined that they were no other vendors that could meet the requirement in accordance with prescribed standards. Market research has determined that this requirement will be Brand Name: Steris Corporation with total small business set-aside.

9. Fair and Reasonable Cost Determination:
I hereby determine the anticipated cost to the Government for this contract action will be fair and reasonable.
Basis for Determination: This procurement will utilize simplified acquisition procedures under FAR Subpart 13.5. The required equipment is commercially available and has been previously procured, which establishes current and historical information that will be used in price analysis as a basis for fair and reasonableness. Certified cost or pricing data will not be required in accordance with FAR 15.4031(b)(3).
