About this file

This is a Bid Submittal Form for a Job Order Contract (JOC) issued by Pinellas County, Florida, in connection with the Fiscal Year 2027 Pavement Preservation Package 2, which encompasses four paving locations consisting of one arterial and three local neighborhood projects totaling approximately 33.27 lane miles of roadway. The contract is structured as an indefinite quantity agreement requiring bidders to submit adjustment factors applied to unit prices contained in a Construction Task Catalog. Bidders must provide a single adjustment factor for each of five work categories: Non-Secure Areas with Normal Working Hours, Non-Secure Areas with Other Than Normal Working Hours, Secure Areas with Normal Working Hours, Secure Areas with Other Than Normal Working Hours, and Non Pre-priced Tasks. All adjustment factors must be expressed to four decimal places and represent either increases or decreases to the base unit prices, with 1.0000 representing net value. Submission of separate adjustment factors for different unit prices will result in rejection of the bid as non-responsive.

The bid evaluation methodology assigns weighted percentages across the five work categories—60% for Non-Secure Areas Normal Hours, 10% for Non-Secure Areas Other Hours, 10% for Secure Areas Normal Hours, 10% for Secure Areas Other Hours, and 10% for Non Pre-priced Tasks—to determine an Award Criteria Figure. Strict mathematical relationships are required between adjustment factors, including that after-hours factors must be greater than or equal to normal-hours factors within the same security classification, secure area factors must be greater than or equal to non-secure factors, and non pre-priced work adjustment factors must equal or exceed 1.0000. The County reserves the right to correct arithmetic errors and confirms that the weighted percentages used for bid evaluation do not guarantee work distribution under the contract in those same proportions.

SECTION F – BID SUBMITTAL FORM

NO CHANGES SHALL BE MADE TO THE FACTORS CONTAINED HEREIN.

ANY CORRECTIONS TO BIDDER ENTRIES SHALL BE INITIALED BY BIDDER.

TO THE COUNTY OF PINELLAS, FLORIDA:

We, the undersigned, hereby declare that no person or persons, firm or corporation, other than the undersigned, are interested in this proposal, as principals, and that this Proposal is made without collusion with any person, firm or corporation, and we have carefully and to our full satisfaction examined the Contract Documents, and that we have made a full examination of the location of the proposed work and the source of supply of materials, and we hereby agree to furnish all necessary labor, equipment, and materials, fully understanding that this is an indefinite quantity Contract, and that we will fully complete all necessary work in accordance with the Contract Documents, within the time limit specified in each Job Order for the unit prices set forth in the Construction Task Catalog®, multiplied by the Adjustment Factors inserted below.

All Unit Prices listed in the Construction Task Catalog® are priced at a net value of 1.0000. The Adjustment Factors shall be an increase or decrease to all the Unit Prices listed in the Construction Task Catalog®. For example, 1.1000 would be a 10% increase to the Unit Prices and 0.9500 would be a 5% decrease to the Unit Prices. Bidders who submit separate Adjustment Factors for separate Unit Prices will be considered non-responsive and their bid will be rejected.

EXAMPLE: Write the Adjustment Factor to four decimal places as the following example illustrates.

1 . 2 1 9 8

OR

0 . 9 9 9 9

For bid evaluation purposes only, the following work distributions shall be used to determine the Award Criteria Figure:

Non-Secure Areas, Other than Normal Working Hours

10%

6. Sum the Extended Total column.

The Sum is the Award Criteria Figure.

1. Specify lines 1 through 6 to 4 decimal places. Use conventional rounding methodology (i.e., if the number in the 5th decimal place is 0-4, the number in the 4th decimal remains unchanged; if the number in the 5th decimal place is 5-9, the number in the 4th decimal is rounded upward).

2. The Other Than Normal Working Hours in Non-Secure Areas Adjustment Factor must be greater than or equal to the Normal Working Hours Adjustment Factor in Non-Secure Areas.

3. The Other Than Normal Working Hours in Secure Areas Adjustment Factor must be greater than or equal to the Normal Working Hours Adjustment Factor in Secure Areas.

4. The Normal Working Hours in Secure Areas Adjustment Factor must be greater than or equal to the Normal Working Hours Adjustment Factor in Non-Secure Areas.

5. The Other Than Normal Working Hours in Secure Areas Adjustment Factor must be greater than or equal to the Other Than Normal Working Hours Adjustment Factor in Non-Secure Areas.

6. The Non Pre-priced Work tasks Adjustment Factor must be equal to or greater than 1.0000.

7. The weighted multipliers (x multiplier) above are for the purpose of calculating an Award Criteria Figure only. No assurances are made by the Owner that Work will be ordered under the Contract in a distribution consistent with the weighted percentages above. The Award Criteria Figure is only used for the purpose of determining the Bid.

8. When submitting Job Order Price Proposals related to specific Job Orders, the Bidder shall utilize one or more of the Adjustment Factors applicable to the Work being performed.

9. The County reserves the right to correct any arithmetic errors for correctness. In the event of an error, the County will use the Adjustment Factors listed in the column titled “Adjustment Factor Bid” to calculate the extended totals and the Award Criteria Figure.
