About this file
This document is a Drug-Free Preference Statement (Bid Form 8C) used in a local government procurement process. The form outlines requirements for businesses bidding on city commodities or contractual services to implement and certify a drug-free workplace program. In the event of identical tie bids regarding price, quality, and service, a business with a certified drug-free workplace program will receive preference in the award process.
The form specifies six key requirements for establishing a drug-free workplace, including publishing a statement prohibiting controlled substance activities in the workplace, informing employees about drug abuse dangers and consequences, providing employees with written documentation of the policy, mandating employee notification of drug-related convictions, imposing sanctions or rehabilitation requirements for convicted employees, and making a good faith effort to maintain a drug-free workplace in accordance with Section 287.087 of Florida Statutes. The document requires an authorized representative's signature, certification of compliance, and notarization to validate the bidding firm's commitment to these standards.

(4) In the statement specified in subsection (1), notify the employees that, as a condition of working on the commodities or contractual services that are under bid, the employee will abide by the terms of the statement and will notify the employer of any conviction of, or plea of guilty or nolo contendere to, any violation of chapter 893 or of any controlled substance law of the United States or any state, for a violation occurring in the workplace no later than five (5) days after such conviction.
(5) Impose a sanction on or require the satisfactory participation in a drug abuse assistance or rehabilitation program if such is available in the employee's community, by any employee who is so convicted.
(6) Make a good faith effort to continue to maintain a drug-free workplace through implementation of Section 287.087, Florida Statutes.
As an authorized representative of the firm, I certify that this firm complies fully with the above requirements.
