About this file
This is a signature page for a backfill contract opportunity issued by the City of Detroit, Michigan. The document establishes the formal submission requirements for contractors bidding on planned residential final grade and site finalization work at five properties located at 19183 Lumpkin, 8435 Siebert, 10157 Woodlawn, 8291 Mt Olivet, and 8273 Mt Olivet. Bidders must submit proposals under their full legal business name, as purchase orders and payments will be issued only in that registered name. The signature page requires contractors to attest that they have visited and inspected all backfill sites prior to submitting their bid, ensuring familiarity with the conditions and scope of work at each property location.
The document mandates that only authorized officers of the firm or individuals with documented authority to commit the firm to the bid may sign the proposal. Contractors must provide their Federal Employer ID number, total number of employees, and the number of employees residing within Detroit. Any assignment of the purchase order, contract, or associated payments requires prior written approval from the Chief Procurement Officer, Chief Financial Officer, and in certain circumstances the City Council. Bidders must include contact information, including email, telephone, and cell phone numbers, along with the authorized signatory's title and position, and identify an alternate company contact for correspondence purposes.
