LITTLE WHITE SALMON NATIONAL FISH HATCHERY
Design/Build Replacement Catwalk and All Associated Appurtenances And Option Tasks if Awarded.
PROPOSAL SCHEDULE
INSTRUCTIONS:
The Offeror shall edit this Proposal Schedule and may provide a detailed breakdown of their Proposal (including support documentation) and all associated line item pricing. It is the responsibility of the Offeror to ensure all requirements are met and all support documentation is provided. The purpose of the Proposal Schedule Document is to organize Base Bid Tasks and Option Bid Tasks for U.S. Fish and Wildlife Service Price Evaluation. Contractor shall show pricing for items. Failure to provide pricing for all items may result in bid not being considered for award.
Design-Build Services - Base and Option Tasks:

Review Existing Site. Prepare and Submit a Summary on the Temporary and Permanent Repair Approach and How it Meets OSHA Standards. Contractor shall consult with a Geotechnical Engineer if required for Connections and/or Construction.

All Associated Electronic and Hard Copy Deliverables.

Construct Temporary Repairs.
To include but not be limited to:
Incidentals.
All Associated Electronic and Hard Copy Deliverables.

The Offeror may provide a detailed breakdown of their bid and all associated line item pricing.
The above Proposal Schedule Description column is not intended to be comprehensive.
It is the responsiblilty of the Offeror to ensure all requirements are met.
As part of the Proposal Schedule, and at the Offeror's discretion, Prepare and Submit Back-Up Documents for the Proposal Schedule including Number of Labor Categories, Labor Rates, Number of Labor Hours, Material Quantities, Equipment Quantities, Equipment Rates, and Required Tools. Include All Items Required and Anticipated to successfully deliver the Project.
As the Contractor is preparing their Proposal, Material Quantities require Contractor assumptions.
The Contractor may submit their assumptions about Material Quantities as part of their Offer.
Whether the Contractor chooses to submit their assumptions about Material Quantities to USFWS for review and as part of their Offer is at the discretion of the Contractor.
