About this file

This document is a Past Performance Questionnaire for a federal contract opportunity for custodial services at Fort Vancouver National Historical Park. The solicitation (140P8325Q0028) involves building 991 custodial services with optional services for building 704 and emergency call-outs, spanning a base period and three option periods. The National Park Service will evaluate potential contractors using a comprehensive performance rating form that assesses technical performance, timeliness, quality, customer satisfaction, cost control, and overall business behavior.

The questionnaire provides a detailed evaluation framework with rating levels of Outstanding, Satisfactory, and Unsatisfactory across eight performance elements. Contractors are required to compile and submit past performance questionnaires as part of their proposal, with the understanding that the government may verify the provided information. Contractors must have active profiles in SAM.gov and submit questions and quotes via email to hal_hoversten@nps.gov. The evaluation will help determine a contractor's suitability for the custodial services contract, with a key decision point being whether the evaluating agency would choose to do business with the contractor again.
