Statement of Work

Project Title: Physical Access Control System Installation

The Bureau of Indian Affairs (BIA), Southern Plains Regional Office located in Anadarko, OK, has a requirement to upgrade the Physical Access Control (PACS). The Regional Office currently has two video/passpoint access/keypad-controlled entries/exits, and one passpoint/keypad-controlled entry/exit (the brand is Altronix). The upgrade requires video/PIV (Personal Identity Verification)/keypad-controlled entries to all entries/exits to the Regional Office which will consist of five (5) entrances total (See attachment of SPRO building layout). This upgrade will allow for the use of PIV cards by employees. The BIA would prefer to have the video intercom Aiphone and GSA APL card reader combined. If not possible, please state in the offer.

The installation of the PACS control panel (GSA APL) shall be required in the designated location.

Utilization of existing components such as wiring, if it’s validated (tested), compatible and properly functional, is allowed. The Contractor will need to install five new PACS Control Readers and hardware on the outside doors of the Regional Office as follows: One (1) South End, One (1) Southwest side, One

(1) East side, two (2) Northwest end, including all required resources for connecting to the new PACS in the designated location. This includes reader licensing and new conduit at the locations where the readers are to be installed. Exit override buttons will be installed in these areas as well. The Contractor shall remove and replace the existing PACS control reader and hardware on the outside of the door of the south, southwest, and east doors. Also, the Contractor shall remove and replace the existing intercom/monitor/doorbell at the south and southwest door and integrate with existing Aiphone KB- 3HRD (5 each) intercom and monitor system or replace with newer Aiphone model, if not compatible.

The PACS shall not restrict emergency exiting from the building. Employees and the public shall be able to exit the building under all circumstances and especially in the event of an emergency, such as a fire or loss of electricity. The existing emergency exit has a rim mounted panic device (push bar) in place. The existing system has not been approved by the local authority having jurisdiction over building safety.

The Contractor shall provide a new workstation computer and monitor in order to install the necessary software for the BIA, Southern Plains Regional Office, to access and make changes to door access configuration and access controls. All devices shall be labeled at the PACS for easy recognition to match provided drawings. Onsite system training for no more than three BIA, Southern Plains Regional Office employees shall be held on the last day of each installation.

The Contractor shall provide all materials, labor, equipment, programming, training and incidentals necessary to install a PACS solution that is Federal Information Processing Standard (FIPS) 201-2 compliant to meet Federal policy, standards and supporting technical specifications. The PACS configuration work shall be performed by personnel that are listed on IDManagement.gov as Certified System Engineer ICAM PACS (CSEIP) trained to ensure the system is installed according to FIPS 201-2 and Federal Information Security Modernization Act (FISMA) requirements. The certification must be current. Only approved products/services from the current Approved Product List (APL) shall be acquired and incorporated into the PACS solution. Visit https://www.idmanagement.gov/approved-products-list/ for more details. The ceilings at the Southern Plains Regional Office are drop ceilings and there is plenty of room to run wiring. The new wiring shall be placed in conduit. It shall be the responsibility of the ePACS contractor to provide electrified door hardware (ie electric strikes, maglocks, etc). Room A-5 shall be the designated ePACS control room (see attached map). If access control cabling is compatible, it may be re-used.

The Contractor shall certify the products and services procured meet all applicable federal standards and requirements, ensure interoperability and conformance to applicable federal standards for the lifecycle of the components, and maintain a written plan for ensuring ongoing conformance to applicable federal standards for the lifecycle of the components. The PACS solution must be capable of supporting approved Public Key Infrastructure (PKI) authentication methods.

Requirements for PKI:

Office of Management and Budget (OMB) Memorandum M-05-24 [M-05-24] provides GSA with the authority to act as executive agent for OMB to ensure that the GSA program serves the needs of the federal enterprise in an inclusive manner to the various standards, requirements, interoperability and conformance as applied within the execution of HSPD-12.

OMB Memorandum M-06-18 [M-06-18] requires all agencies to use the General Services Administration (GSA) Approved Products List [APL] in all procurements for Homeland Security Presidential Directive 12 [HSPD-12] and Federal Information Processing Standard 201 [FIPS 201] compliant systems. The GSA FIPS 201 Evaluation Program lists compliant Enterprise Physical Access Control Systems (E-PACS) solutions on the [APL] for procurements.

OMB Memorandum M-11-11 [M-11-11] requires all E-PACS systems at federally owned or leased buildings “be enabled to use PIV credentials” and that “agency processes must accept and electronically verify PIV credentials issued by other federal agencies.” [M-11-11] further requires compliance with FICAM Roadmap and Implementation Guidance [Roadmap], which expands the use of Personal Identity Verification (PIV) to include PIV Interoperable (PIV-I) credentials in FICAM compliant solutions. Both PIV and PIV-I credentials contain PKI digital certificates, fingerprint minutia templates, and a secure Personal Identification Number (PIN).

The PACS shall be installed and tested to ensure all components are fully compatible as a system and can be integrated with all associated and existing ACS and security subsystems. The contractor shall provide final drawings of the PACS solution to include:

• Identify how all equipment within the PACS, from main panel to device, shall be laid out and connected.

• Provide full detail of all system components wiring from point-to-point.

• Identify wire types utilized for connection, interconnection with associated security subsystems, if applicable.

The final drawing shall match all device identification as noted on actual devices for easy recognition for future reference. Provide the following information:

• Device ID

• Device location

• Mounting type

• Power supply or circuit breaker and power panel number

• Door locations

Tests and Training:

• All testing and training shall be compliant with the general requirements

• Provide training services for operation and maintenance of units.

Warranty: All components, parts and assemblies provided by the manufacturers and installed by the Contractor shall be warranted against defects in material and workmanship for a period of 12 months (parts and labor), commencing upon date of acceptance by the Bureau of Indian Affairs, Southern Plains Regional Office.

Project Title: Security Alarm Upgrade- W.C.D. Office Building / Bureau of Indian Affairs, Southern Plains Regional Office

Introduction

The W.C.D. / BIA SPRO office building is soliciting proposals to upgrade its fire alarm system. The existing system is an ADEMCO combination fire and security alarm is 15+ years old. The existing security alarm system has begun having issues with false alarms and due to the age, parts and devices are longer available. The WCD/SPRO office building is 30,692 sq ft and was built in 1975.

Security Alarm System Installation A new security alarm/ intrusion detection systems shall be installed in accordance with the requirements of UL 681 Installation and Classification of Burglar and Hold Up Alarm Systems.

Contractor Qualifications

1. Contractor's Business License: A copy of the Contractor's valid State of Oklahoma License. The contractor must be licensed in the state of project location and have been incorporated in the business in that state for a minimum of five (5) years.

2. Proof of Experience: Proof that Contractor has successfully installed similar systems for security alarm /intrusion detection components on previous projects of comparable size and complexity.

3. Service Capability: The security alarm contractor shall have in-house technician or local Engineer, installation and service personnel with a maintenance office within 50 miles of the project location.

4. Equipment Manufacturer Authorization Letters: Letters from the (all) equipment manufacturer(s) stating that the Contractor is a Factory Authorized Distributor and Installer, and is trained and certified for the equipment proposed on this project and is licensed to purchase and install the software required to provide the specified functions.

5. Certifications:

a. Documentation that the Contractor has on staff personnel factory-trained and certified for the equipment proposed for this project.

6. Contractor technicians shall be licensed and certified in accordance with applicable state and local rules and regulations governing commercial companies providing security systems and services.

7. Contractor shall acquire any low voltage permits required by State or local codes and provide copy to the COR.

Scope of Work

The contractor shall furnish all labor, equipment, incidentals, software, hardware, programming, supervision, and transportation for work necessary to supply and properly install a fully functional security system and provide 24/7 monitoring services.

The new security alarm system and components shall be UL listed. The contractor will be required to provide all materials, devices, workmanship, and programming to complete the job to the Bureau of Indian Affairs COR satisfaction. This includes, but not limited to; control panel/communicator, Fixed English display keypads, power supplies, modules, battery backup, door contacts, surge protection, PIR motion detectors, ethernet drops, wiring [electrical and, devices, alarm/siren, and all related appropriate field devices. The contractor will provide all necessary test equipment and labor to perform required inspection, testing, maintenance, and repairs.

Contractor is responsible to field verify all site conditions and information provided is accurate before commencing any portion of their work under this SOW.

Equipment not properly installed (i.e., to manufactures specifications and to the satisfaction of the government) shall be corrected at no additional cost to the client.

Deliverables/Requirements

Contractor shall provide and install all equipment as needed to provide a fully functional security alarm system along with 24 hours a day, 7 days a week security monitoring services to include call outs to Government personnel, local law enforcement, BIA Police, and emergency personnel when needed.

Contractor to provide up to a one (1) hours block for training for WCD and Government personnel. The security system shall meet the following:

AC Power Requirements

All AC power wiring and work for 110/120 VAC circuits shall be performed by licensed electrician or approved licensed electricians. Contractor will furnish all conduit, electrical wiring, circuit breakers, outlets, terminations, etc., needed to accomplish a complete functioning AC circuit for security alarm system as required.

All power to the security alarm equipment, panels, and power supplies shall be hardwired directly to the powered device. The licensed electrician shall connect the power supply.

Backup Power Supply

Provide appropriate backup power supply with minimum of 24-hour standby.

Labeling

All devices, wiring, keypads, and panels shall be labeled at all terminal and junction locations.

Equipment Removal and Disposal Requirements

The contractor is responsible for removing and disposing of the existing security alarm system, devices, components, and wiring etc. and shall be removed back to the source. Contractor is responsible for legal disposal of all materials, components, waste, debris in accordance with all federal, state, and local laws or regulations. It shall be the responsibility of the subcontractor to remove their debris from the project site and, on a daily basis, dispose of it properly. Work areas will be kept neat and orderly at all times. All packaging debris shall be removed and disposed of off-site.

Replacement of ceiling tiles required for locations where existing equipment was removed and not replaced. Match existing ceiling tiles close as possible. Blank cover plates to be installed over existing openings/j-boxes in walls to be abandoned with color to match to wall as close as possible. Larger opening in walls shall be patched and filled in to match existing location as required.

Equipment and Installation

All materials and workmanship provided by contractor shall be in accordance with the best practice of state of the art technical security system installation trade conventions. All work to be installed in accordance with the manufacture’s specifications and the correct power is provide to each device/item.

It shall be the contractor’s responsibility to obtain the necessary installation specifications from the manufacture. The government shall not furnish any manufacture’s information except that which is included in the contract documents.

The security system components shall be commercial grade configured to industry and UL standards. All equipment, devices, and wiring must be new. No grey market remanufactured or refurbished products.

• Security system shall include 24/7 monitoring of access control and intrusion detection into one system. The control communicator panel and siren shall be hard wired while all of the support elements can be hard wired or wireless technology.

• The keypad shall have system status displayed in English with optional AC power supply and illuminating keys.

• The siren shall output a minimum of 95 decibels at 12 DC volts capable of producing steady and warble tones.

• The door contact shall be weather-proof, water resistant with magnet/magnetic spacer [wireless contacts will be powered by lithium batteries].

o In case of dual door contacts (i.e. double doors) the contacts shall be wired as a single point of alarm.

• The motion sensor/detector shall include adjustable sensitivity levels.

Testing/ System Acceptance

Testing and acceptance shall be performed at the completion of all installations. Upon completion of the security alarm system installation, the contractor shall perform a system walk thru and thorough system test, in accordance with NFPA 731, shall be witnessed and accepted by local government representative. Once completed, the contractor shall orient (provide training to) the government and WCD Facilities personnel about the security alarm system and its features.

Training

Once installation has been completed and system has been tested and accepted by government, the contractor shall provide up to one (1) hour block for the training/orientation to Government and WCD Facilities Personnel. Original Equipment Manufacture’s (OEM) manuals, warranty information, and any additional information that will assist in understanding and operating the system shall be provided to the COR.

Service Monitoring

The central station monitoring facility must be located in the Continental United States. The central facility shall perform dispatches in accordance to American National Standards Association (ANSI) /

Central Station Alarm Association (CSSA CS-V-01). The contractor shall make available copies of all the National Recognized Testing Laboratories (NRTL) Certificates and all government COR or representative personnel access to the central station service provider’s facility for inspection prior to contract award, and any other time deemed necessary by the COR throughout the life of this contract. The central station shall be responsible for the monitoring of all active security systems under this contract. The system must be grouped and segregated from general accounts in order to maintain accountability of personnel access and alarm event processing. Provide 24 hours a day, 7 days a week security monitoring service. This shall include notification to Government personnel, local law enforcement, BIA Police, and emergency personnel when needed.

• Failures in Systems and Communication: The Contractor shall test for, act upon and resolve all failures in the security system including setting adjustments needed for false alarms due to bugs, rodents, wind, etc. The government will be responsible for repairing damaged phone lines. The Contractor shall act upon each communication failure by utilizing the emergency contact list provided after contract award, with the exception of notifying local law enforcement. If no contact is established, a voice mail message is to be left when available, and the Contractor shall continue through the contact list until all contact numbers have been utilized. The Contractor shall resolve each communication failure by contacting the government and scheduling a service call for a technician to determine the cause of the communication failure, and to restore communication when the Contractor’s equipment causes the failure.

• Emergency Notifications: Upon receipt of any and all alarm signals, the Contractor is to notify the individual(s) listed on the Government provided emergency contact list. The Government’s emergency contact list will be provided after contract award and will be updated when needed. If an emergency exists, the Contractor shall dispatch local law enforcement and notify the Government.

Service Interruption

Contractor technicians shall be properly equipped to service and repair systems with minimal down time. If the system equipment cannot be fully restored upon initial response, the contractor shall repair/restore the system no later than (NLT) 3 days after the initial report of failure. If replacement equipment is not available, the Contractor shall provide loaner equipment until a permanent fix is accomplished. A permanent fix with replacement equipment shall be accomplished NLT 30 days after the initial response to the trouble call.

Liability and Damages to the Government

Liabilities:

The contractor assumes all responsibilities and liabilities involved in obtaining the proper installation permits and obeying state, county and local laws during the installation of security system.

Licensed:

The Contractor technicians shall be licensed and certified in accordance with applicable state and local rules and regulations governing commercial companies providing security systems and services.

Responsibility for Damages:

The Contractor shall cooperate with Government personnel in safeguarding Government property. The Contractor shall be responsible for reporting all acts of vandalism or peril.

Safety Notwithstanding any other provision of this contact, the contractor shall comply with all safety and health requirements as outlined in Safety Standards to ensure safe working conditions for employees and government workers.

Asbestos

Asbestos is present in the WCD/ SPRO office building. Locations have been identified and is localized in E Section of the building joint compound throughout this area. Additional known locations include transite siding installed in soffits over each entrance to the building and on exterior of E Section above each window. Asbestos Containing Material (ACM) floor tile and mastic are present throughout the entire WCD SPRO building. Asbestos sampling report is maintained on site at the WCD/SPRO building in the Safety Managers office and is available upon request.

Disturbance of Asbestos and Asbestos Contain Materials (ACM) shall only be performed by trained and accredited abatement contractor that is currently licensed by State of Oklahoma. Disturbance of ACM without following proper safety regulations and procedures identified by OSHA 29 CFR 1910.1000, 29 CFR 19126.1101, and Oklahoma Asbestos Control Act 40 O.S. § 450, Abatement of Friable Asbestos Materials Rules OAC 380:50. Accidental and willful disregard of this requirement will result in contractor being responsible for any and all cost for emergency cleanup operations by an accredited licensed abatement contractor. Air monitoring and clearance samples are required for all ACM disturbing activities.

Verbal notification shall be provided to BIA COR and Regional Safety Manager at a minimum of 24 hours prior ACM disturbing activities. Any accidental damage or disturbance of ACM contractor will notify Regional Safety Manager immediately.

Final Inspection

Contractor shall submit all certifications and documentation, as-builts as applicable, at least 14 calendar days prior to the proposed final inspection date(s). Coordination with Government COR, to conduct final inspection and testing for all installed equipment and devices per NFPA 731 and UL requirements.

Acceptance of the Installation

Acceptance of the installation shall be granted only after the following conditions have been met:

1. All work has been completed in a satisfactory manner, in accordance with this SOW.

2. Performance of the technical security system has been demonstrated in a satisfactory fashion.

3. BIA has received all required documentation and as-built drawings, The contractor shall provide a final test document (listing each device) that indicates the system has been tested and is functioning as one complete security alarm system, according to the manufacture’s specifications and the government’s satisfaction.

Ownership of Equipment and Materials

After completion and government acceptance, all materials furnished by the contractor shall become the sole property of the government.

Completion and Close Out

Project Title: Fire Alarm Upgrade- W.C.D. Office Building / Bureau of Indian Affairs, Southern Plains Regional Office

Introduction

The W.C.D. / BIA SPRO office building is soliciting proposals to upgrade its fire alarm system. The existing system is an ADEMCO combination fire and security alarm is 15+ years old. The existing fire alarm system does not provide adequate coverage or notification to building occupants and has begun having issues with false alarms and due to the age, parts and devices are longer available.

Fire Alarm System Installation New fire alarm systems shall be installed in accordance with the requirements of NFPA 72, NFPA 70, NFPA 5000, and the appropriate GSA fire alarm system specification.

Contractor Qualifications

Equipment Manufacturer Authorization Letters: Letters from the (all) equipment manufacturer(s) stating that the Contractor is a Factory Authorized Distributor and Installer, and is trained and certified for the equipment proposed on this project and is licensed to purchase and install the software required to provide the specified functions.

5. Certifications:

a. Provide a copy of the National Institute for Certification in Technologies (NICET) Technician

Level 3 Certificate for the project Engineer actively proposed for the project.

b. Documentation that the Contractor has on staff personnel factory-trained and certified for the equipment proposed for this project.

6. Contractor technicians shall be licensed and certified in accordance with applicable state and local rules and regulations governing commercial companies providing security systems and services.

7. Contractor shall acquire any low voltage permits required by State or local codes and provide copy to the COR.

Scope of Work

The contractor will design and install a new addressable fire alarm and detection system that will meet all current NFPA, state, and local code requirements. The new system will be UL and FM approved. The contractor will be required to provide all materials, devices, workmanship, and programming to complete the job to the Bureau of Indian Affairs Department of Risk and Safety Managements [DSRM] satisfaction. This includes, but not limited to; control panels, power supplies, modules, battery backup, magnetic door releases, surge protection, ethernet drops, wiring, conduit, devices, pull stations, fire alarm control panels, horn strobes, and all related appropriate field devices.

The fire alarm system and components shall be commercial grade configured to industry and UL standards. All equipment, devices, and wiring must be new. No grey market remanufactured or refurbished products.

The contractor will provide all necessary test equipment and labor to perform required inspection, testing, maintenance, and repairs.

The new system shall identify the alarmed unit and its location precisely to facilitate the location of the fire.

The existing fire alarm will be maintained operational as long as feasibly possible while the system is being replaced. Notification shall be provided to WCD/BIA 48 hours prior to taking the existing fire alarm system offline and out of service.

Contractor is responsible to field verify all site conditions and information provided is accurate.

Special Requirements: The design requirements below supersede the requirements of NFPA 72 and the

NFPA 5000:

• All new and replacement fire alarm systems shall be an addressable system as defined in NFPA 72.

• The alarm communication system shall provide an automatic response to the receipt of a signal indicative of a fire emergency. Manual control capability shall also be provided to notify all occupants either on a selective or all-call basis during an emergency.

• Fire alarm systems shall not be integrated with other building systems such as building automation, energy management, security, etc.

- Fire alarm systems shall be self-contained, standalone systems able to function independently of other building systems.

• Wiring supervision for fire alarm systems shall be provided as defined in NFPA 72 as follows: – Interconnected riser loop or network (Style 7 – Class A) – Initiating device circuits (Style B – Class B)

- Signaling line circuits (Style 4 – Class B)

- Signaling line circuit for network (Style 7 – Class A)

- Notification appliance circuits (Style Y – Class B)

• All fire alarm system wiring shall be solid copper and installed in conduit. Stranded wiring shall not be used.

- Conduit shall be rigid metal or electrical metallic tubing, with a minimum inside diameter of

3/4 inch, that utilizes compression type fittings and couplings.

- Conduit, connectors, j-boxes installed above walls and drop tile ceilings shall be red in color.

- Wiring type and size to be in accordance with NFPA 70, Article 760

AC Power Requirements

• All AC power wiring and work for 110/120 VAC circuits shall be performed by licensed electrician or approved licensed electricians. Contractor will furnish all conduit, electrical wiring, circuit breakers, outlets, terminations, etc., needed to accomplish a complete functioning AC circuit for security alarm system as required.

• All power to the fire alarm equipment, panels, and power supplies shall be hardwired directly to the powered device. The licensed electrician shall connect the power supply.

Manual Fire Alarm Stations Manual fire alarm stations shall be installed in accordance with the requirements of NFPA 72 and the NFPA 5000. Manual fire alarm stations shall be single-action.

Special Requirements. The design requirements below supersede the requirements of NFPA 72 and the

NFPA 5000:

Smoke Detectors

Smoke detectors shall be installed in accordance with the requirements of NFPA 72, NFPA 90A, and the

NFPA 5000.

Carbon Monoxide Detectors

Carbon Monoxide detectors to be installed in Mechanical/HVAC rooms where natural gas fueled HVAC and water heaters are located.

Additional Devices

Include wiring, conduit, and connection to the three existing FireTrace self-contained fire suppression systems in the Lektriever file sorting systems located in LTRO. Installation of additional drops for additional FireTrace units to be added in the future [5 in LTRO and 2 HR].

Audible Notification Appliances

Performance, location, and mounting of audible notification appliances shall be in accordance with the requirements of NFPA 72.

Backup Power Supply

Provide appropriate backup power supply with minimum of 48-hour standby.

Special Requirements.

The design requirements below supersede the requirements of NFPA 72:

• To ensure audible signals are clearly heard, the sound level shall be at least 70 dBA throughout the office space, general building areas and corridors measured 5 feet above the floor. The sound level in other areas shall be at least 15 dBA above the average sound level or 5 dBA above any noise source lasting 60 seconds or longer.

• The design for the placement and location of all audible notification appliances shall be based on the applicable calculation methods contained in The SFPE Handbook of Fire Protection Engineering for calculating sound attenuation through doors and walls.

Visible Notification Appliances

Placement and spacing of visible notification appliances shall be in accordance with the requirements of

NFPA 72.

Special Requirements. The design requirements below supersede the requirements of NFPA 72:

• Visual notification appliances shall only be installed in projects that involve the installation of a new fire alarm system.

• Visual notification appliances shall only be required to be installed in public and common areas.

Public and common areas include public rest rooms, reception areas, building core areas, conference rooms, open office areas, etc. For the purposes of this requirement, visual notification appliances shall not be required to be installed in individual offices.

• Visual notification appliance circuits shall have a minimum of 25 percent spare capacity to accommodate additional visual notification appliances being added to accommodate employees who are deaf or have hearing impairments.

Graphic Annunciator Fire alarm systems shall have at least one graphic annunciator located at the entrance to the building that the fire department enters. [South Entrance]

Labeling

All devices, wiring, panels, j-boxes, and pull stations shall be labeled at all terminal and junction locations.

Fire Alarm Control Panel (FACP) label shall include electric panel number, electrical source, from which the panel is fed, NACs address.

Circuit breaker providing power to FACP shall be labeled with red sticker indicating ‘FIRE ALARM” and E-lockout kit circuit breaker lock attached.

Submittal and Review

The awarded contractor will be required to provide AutoCAD drawings for the project as well as three (3) sets of full-size blueprints. The drawings will be reviewed by the office of the Department of Interior (DOI), Bureau of Indian Affairs (BIA), Department of Risk and Safety Management (DSRM), for approval before the project can begin. For this scope of work, DSRM is the authority Having Jurisdiction (AHJ).

Equipment Removal and Disposal Requirements

The contractor is responsible for removing and disposing of the existing fire alarm system(s), devices, components, smoke detectors, and wiring etc. and shall be removed back to the source. Contractor is responsible for removal and legal disposal of all materials, components, waste, debris in accordance with all federal, state, and local laws or regulations. Replacement of ceiling tiles required for locations where existing equipment was removed and not replaced. Match existing ceiling tiles close as possible.

Blank cover plates to be installed over existing openings/j-boxes in walls to be abandoned with color to match to wall as close as possible. Larger opening in walls shall be patched and filled in to match existing location as required.

Service Monitoring

Provide 24 hours a day 7 day per week monitoring of fire alarm notification system. This shall include notification to Government personnel, Anadarko Fire Department BIA Police, and emergency personnel when needed.

Training:

Once installation has been completed and system has been tested and accepted by government, the contractor shall provide up to two (2) hour block for the training/orientation to Government and WCD Facilities Personnel.

Safety Notwithstanding any other provision of this contact, the contractor shall comply with all safety and health requirements as outlined in Safety Standards to ensure safe working conditions for employees and government workers.

Asbestos

Asbestos is present in the WCD/ SPRO office building. Locations have been identified and is localized in E Section of the building joint compound throughout this area. Additional known locations include transite siding installed in soffits over each entrance to the building and on exterior of E Section above each window. Asbestos Containing Material (ACM) floor tile and mastic are present throughout the entire WCD SPRO building. Asbestos sampling report is maintained on site at the WCD/SPRO building in the Safety Managers office and is available upon request.

Disturbance of Asbestos and Asbestos Contain Materials (ACM) shall only be performed by trained and accredited abatement contractor that is currently licensed by State of Oklahoma. Disturbance of ACM without following proper safety regulations and procedures identified by OSHA 29 CFR 1910.1000, 29 CFR 19126.1101, and Oklahoma Asbestos Control Act 40 O.S. § 450, Abatement of Friable Asbestos Materials Rules OAC 380:50. Accidental and willful disregard of this requirement will result in contractor being responsible for any and all cost for emergency cleanup operations by an accredited licensed abatement contractor. Air monitoring and clearance samples are required for all ACM disturbing activities.

Verbal notification shall be provided to BIA COR and Regional Safety Manager at a minimum of 24 hours prior ACM disturbing activities. Any accidental damage or disturbance of ACM contractor will notify Regional Safety Manager immediately.

Spare Parts and Equipment

Contractor shall provide minimum of 10 percent of total amount of installed devices, expendables and consumables are required to be delivered at completion of install. This includes pull station, smoke detectors, heat detectors, CO detectors, audible/visual notification devices.

Final Inspection

Contractor shall submit all certifications and documentation, as-builts as applicable, at least 30 calendar days prior to the proposed final inspection date(s). Coordination with DSRM, or authorized representative, to conduct final inspection and testing for all installed equipment and devices per the National Fire Protection Association requirements.

Acceptance of the Installation

Acceptance of the installation shall be granted only after the following conditions have been met:

1. All work has been completed in a satisfactory manner, in accordance with this SOW.

2. Performance of the technical security system has been demonstrated in a satisfactory fashion.

3. BIA has received all required documentation and as-built drawings, The contractor shall provide a final test document (listing each device) that indicates the system has been tested and is functioning as one complete security alarm system, according to the manufacture’s specifications and the government’s satisfaction

Ownership of Equipment and Materials

After completion and government acceptance, all materials furnished by the contractor shall become the sole property of the government.

Completion and Close Out
