About this file
This is a Hot Work Permit form (Form HW-1) issued by the National Park Service for authorizing and controlling hot work operations at federal facilities.
The form establishes mandatory permitting requirements for hot work activities, which may only be performed in designated areas or permit-required areas that have been inspected and approved by a Permit Authorizing Individual. Hot work is prohibited in non-authorized areas, sprinklered buildings with impaired protection, areas with explosive atmospheres, improperly prepared containers that previously held flammable materials, and areas with combustible dust accumulation. The form requires documentation of the work location, description of work to be performed, contractor information, NPS employee oversight details, and special precautions. Required safety precautions include ensuring sprinklers are in service, equipment is in good repair, floors within 35 feet are clean and free of combustibles, all wall and floor openings are covered, and combustibles are moved away from work surfaces. For enclosed equipment work, containers must be cleaned and purged of flammable vapors. A fire watch must be provided during operations and for 60 minutes afterward, with personnel equipped with a fully charged fire extinguisher and trained in emergency procedures. The work area and adjacent areas must be inspected every 20 minutes for one hour after completion to confirm fire safety. The Permit Authorizing Individual must sign and date the permit, specifying the issue and expiration dates, and indicate whether a fire watch is required.
