About this file
This is a Past Performance Questionnaire form (Attachment 5) for National Park Service solicitation 140P2125Q0008, used to evaluate consultants bidding on a contract to assist the Commercial Services Program with concession contract prospectus development.
The questionnaire requires evaluators to rate consultants' previous performance in areas including project due diligence, market and demand analyses, rate comparability studies, personal property valuations, replacement schedules, financial investment analyses, and prospectus document drafting for hospitality and recreation services in national parks. The form uses a 5-level rating scale (Exceptional, Acceptable, Marginal, Unacceptable, No Rating) to assess both technical performance (ability to meet requirements/schedules, quality of deliverables) and management approach (project management effectiveness, personnel management, subcontractor oversight). Completed questionnaires must be submitted to christina_gingras@nps.gov by February 27, 2025. The document is designated as source selection sensitive and restricted to evaluation team use only.

Attachment 5 – Past Performance Questionnaire National Park Service Solicitation 140P2125Q0008
The National Park Service is in the process of selecting consultants to assist our Commercial Services Program in the development and analysis of concession contract prospectuses. Among the services the successful consultant must perform under the published solicitation are project due diligence, market and demand analyses, rate comparability studies, personal property valuations and replacement schedules, financial investment analyses, and drafting prospectus documents for hospitality and recreation services provided to visitors in national parks.
As required by the Federal Acquisition Regulation (FAR) at paragraph 12.206, each consultant’s past performance history will play an important role in determining which consultants will receive a contract to provide these services.
As someone with first-hand knowledge of the past performance history pertaining to one of the consultants preparing a bid for a prospectus development contract, your participation is essential. We depend on your critical evaluation of the prospective consultant’s performance in order to ensure we obtain high quality expertise and professionalism.
To assist you in preparing your response and expediting your reply, we have formulated the attached questionnaire highlighting the areas of primary interest to us. Please provide information specific to particular projects performed under those contracts or through collaborative relationships rather than past performance information pertaining to overarching contracts or long-term collaborative relationships that cover multiple projects or initiatives (like an IDIQ contract or a history of providing a type of service to a particular client).
To assist our timely decision, we need your written response no later than close of business on 2/27/2025.
You may complete the questionnaire by hand and email to christina_gingras@nps.gov.
If you have any questions concerning completion of the attached questionnaire, please contact Christina Gingras at christina_gingras@nps.gov.
When completed, this document is source selection sensitive and the Service will use it only for evaluation of this proposal and not share it outside of the evaluation team.
