About this file

This document is a Past Performance Questionnaire (PPQ) from the U.S. Department of Interior, National Park Service for the GATE – Rehab Jacob RIIS Ship Rail project (Solicitation Number 140P4525R0030). The form is designed to collect detailed performance feedback about a contractor's previous project work. It provides a structured evaluation process where the contractor completes Section A with project details, then sends the form to a previous project owner/evaluator to complete Section B, which includes ratings across multiple performance categories such as relationship management, corporate integrity, quality control, and schedule adherence. The questionnaire uses a rating scale from Outstanding (O) to Unacceptable (U) and includes questions about customer satisfaction, potential negative performance issues, and whether the evaluated firm would be recommended for future contract work.
