About this file

This is a Past Performance Questionnaire template designed for the GRTE Taggart Lake Area Renewal project (Solicitation 140P1426R0003), a $5-10 million highway and bridge construction solicitation with the National Park Service Northern Rockies region. The questionnaire is intended to be completed jointly by offerors and their past project references to provide source selection information regarding contractor performance history.

The questionnaire requires offerors to provide contractor information, contract details, and project scope in Sections A and B, then transmit to reference clients who complete Sections C and D with performance ratings. References are evaluated across five key performance areas using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory): Quality (workmanship, quality control, technical submittals); Schedule and Time Management (milestone achievement, responsiveness, timeliness); Cost Control (site condition management, modification pricing, invoice accuracy, subcontractor payments); Management (project oversight, subcontractor coordination, safety, flexibility, owner relationships); and Small Business Subcontracting and Labor Standards (regulatory compliance, small business utilization). Additionally, references must confirm regulatory compliance with building codes and provide an overall recommendation regarding the contractor's suitability for similar future contracts. Completed questionnaires must be returned to Molly Lynam, Contract Specialist with the National Park Service Northern Rockies MABO, at molly_lynam@nps.gov.
