NEW FORM EFFECTIVE 07/10/2020. This NEPA Environmental Checklist (EC) is used to determine the environmental requirements associated with a proposed project. Complete the first 2 pages of the form, attach a site map and a SOW or 1391, and then submit to the PWD Environmental Division (EV). A complete EC with required attachments should be submitted a minimum of 15 days prior to project’s scheduled start date. If the project scope is modified or if project work has not been initiated within 1 year of the signature date, please contact EV for review/approval prior to initiating project activities.

ADDITIONAL REQUIREMENTS or COMMENTS Comment 1 The proposed action is within the scope of CATEX #14: Alterations of and additions to existing buildings, facilities, and systems (e.g., structures, roads, runways, vessels, aircraft, or equipment) when the environmental effects will remain the substantially the same and the use is consistent with applicable regulations. None of the exclusions from reliance on a CATEX applies in this case. A Record of Categorical Exclusion is required, but further NEPA analysis is not warranted. The applicability of this CATEX determination is based on the full implementation of all environmental requirements identified below.

Comment 2 A Record of Non-Applicability was completed to document compliance with the General Conformity Rule, Section 176(c) of the Clean Air Act. The proposed action is exempted from a conformity analysis as the emissions will not exceed the de minimis ozone threshold level and the emissions are not considered regionally significant.

Requir 1 PW NLON EV Division shall be notified immediately of any actions that require dewatering activities under the scope of this project, including drain-down of any water piping and equipment. Discharge to ground and/or stormwater system is prohibited. Approved discharges to sanitary are subject to the General Permit for the Discharge of Wastewaters from Significant Industrial Users (SIU GP). No discharge shall occur without prior approval from the PW NLON EV Division Wastewater Program Manager (x2525). The contractor may be required to obtain separate permitting and provide monitoring data prior to discharge.

Requir 2 Discharges from the cooling towers at B533 are currently permitted under the Comprehensive Discharges to Surface Water and Groundwater General Permit (COMP GP). All discharges resulting from changes, replacement, modifications, etc., to these cooling towers shall comply with the discharge limits of the COMP GP. As the preferred alternative, PWD EV recommends that this project include rerouting the discharge location to the sanitary sewer and re-permitting under the Significant Industrial Users General Permit (SIU GP). Discharge limits are less stringent under the SIU GP.

Requir 3 Existing facility HAZMAT surveys shall be reviewed prior to any facility disturbance. If suspect hazardous materials (such as asbestos and lead paint) are present and are anticipated to be disturbed under the scope of the project, surveys shall be conducted by qualified individuals in advance of construction activity. All survey results shall be provided to the PW NLON EV Division.

Requir 4 A list of HAZMAT scheduled for use or installation on the project and the approximate quantities/volumes of these materials shall be submitted to the PW NLON EV Division EPCRA Manager (x5140) prior to initiating project activities.

Requir 5 A backflow prevention device shall be required for any connections to potable water lines. All work involving the testing and certification of backflow prevention devices or surveys for cross-connections in the water distribution system shall be done in accordance with DoD and Navy regulations and policies, CT Department of Public Health (CTDPH) requirements, and American Water Works Association standards and practices. Information shall be provided to the PW NLON EV Division Drinking Water Manager (x5140) regarding the testing performed on any such devices and the CTDPH-certifications of the person(s) performing these tests.

Requir 6 Appropriate disinfection procedures shall be performed in association with any work conducted on the fresh water supply lines in accordance with Section 19-13-B47 of the CTDPH regulations. This work shall be done using established procedures accepted by the American Water Works Association and copies of all test results confirming the successful completion of disinfection procedures shall be forwarded to the PW NLON EV Division Drinking Water Manager (x5140).

Requir 7 Disposal of any regulated waste shall be coordinated in advance with the PW NLON Hazardous Waste Program Office (x4298).

- Non-regulated solid waste (i.e., general trash, construction debris, and recyclable items) shall be properly managed and disposed of in accordance with Federal, State, Navy, and local regulations.

- Any regulated waste generated under the scope of this project shall be managed, stored, and disposed of in accordance with the SUBASENLON Hazardous Waste Management Plan.

Requir 8 Use of exterior storage and laydown areas in support of this project shall be done in compliance with SUBASENLON instructions and policies. Site must be evident of good housekeeping practices consistent with proper storm water management and control.
