About this file
This is an Invitation for Bid (IFB) form submitted as an offer to the State of Illinois for solicitation B-49751, related to JPMC Chicago Metro Presort Mail services. The document serves as the formal bid submission template that vendors must complete and submit to respond to the state's solicitation. The form requires bidders to confirm their understanding of all required documents outlined in solicitation section A.1 and acknowledges that failure to meet requirements constitutes grounds for disqualification. The document does not specify quantities, response dates, due dates, site visits, or bidders' conferences within the provided content, as the form appears to be a template rather than a fully populated solicitation notice.
The document addresses Illinois Procurement Code preferences available to bidders, including Illinois Business Preference and Domestic Product Preference, with vendors required to complete appropriate affidavits or designations to claim such preferences. Bidders may take exceptions to Standard Terms and Conditions by submitting a redline version with their offer, subject to state acceptance or rejection. The Standard Terms and Conditions are referenced as available on the Illinois Central Procurement Office website. The form includes signature and date fields for authorized representatives, and all checkbox fields appear unchecked in the provided document, indicating this is a blank template awaiting completion by responding vendors.
