The form is specifically designed for solicitation FA251825r0029 issued by the Department of the Air Force Space Command, focusing on verification/re-verification testing and hardness sustainment of NORAD and NORTHCOM HEMP systems at Peterson Space Force Base in Colorado Springs. Contractors are required to complete one form per functional area, identifying actual or potential organizational conflicts of interest, and if applicable, provide detailed mitigation strategies. The form must be submitted in Microsoft Excel format directly to the Contracting Officer, following specific naming conventions and navigation instructions.
