PWS ATTACHMENT B - DATA REQUIREMENTS

Provide the data as indicated below. All reports, submittals, logs and information provided to the Government shall be electronic and compatible with Microsoft Office, such as Excel, Word, and others.

“Compatible” means the document can be opened using the appropriate Microsoft Office component without any discernible affect on the text (e.g., loss of data) or the formatting of the document.

Schedule identifies month that service will occur. (PWS, paras 1.1)

Non-permanent schedule changes beyond contractor’s control (PWS para 1.3)

Contractor Quality Control Plan

Plan addresses contractor’s procedures to identify, prevent, and ensure non-recurrence of defective services (PWS, para 2.1.)

DD Form 2772, Contract Discrepancy Report (CDR)

Issued to the contractor when a performance threshold has not been met or contractor performance has not been accomplished (PWS, para

Progress Meeting Minutes

If contractor does not concur with the minutes, provide a written notification identifying areas of non-concurrence for resolution (PWS, para 2.4)

Maintain updated list of names and telephone numbers of contractor’s on-site manager and alternate employees (PWS, para 4.1.1)

Applications of authorized contractor employees requiring access to CAFB (PWS, para 4.2.3)

Provide to CO, who may be required to coordinate on applications of authorized contractor employees requiring access. Applications will be processed by Pass & ID Section (Welcome Center)

Terminated Employees

Identification (ID) cards and base passes issued by Security Forces (PWS, para 4.2.3)

Turn in ID cards and base passes to Security Forces (Welcome Center) upon termination of contractor employee

Mishap Notification and Reporting

Mishaps involving on-duty contractor employees, Air Force assets, or Air Force operations which result in death, injury, illness, occupational disease, adverse environmental impact or damage to Government property (PWS, para 4.3, inclusive)

Each occurrence; make notification in most expedient means; provide copy of mishap reports within 5 days of incident

Most expedient means to COR; if COR not available, notify CO

Environmental Management System (EMS)

EMS Awareness Training (PWS, para 4.4)

Phase Out Plan

Plan addresses continued service by incumbent contractor without interruption upon contract expiration (PWS, para 4.8)
