About this file
This document is a Blanket Purchase Agreement (BPA) Terms and Conditions for comprehensive audiometric testing services. The BPA will be in effect from August 1, 2025, through July 31, 2030, or until total orders reach $250,000, with a maximum individual call order limit of $25,000. The agreement requires the contractor to provide full audiometric testing to determine the type and degree of hearing loss, with pricing to be updated annually and submitted to the Contracting Officer.
Key requirements include using Wide Area Workflow (WAWF) for payments, maintaining a 48-hour cancellation notification window, ensuring contractor employee security and identification, and submitting itemized invoices after completing all scheduled exams. The 934th Aeromedical Staging Squadron will manage the testing, with specific authorized ordering officers responsible for scheduling and coordinating audiometric tests. The government is exempt from state and local sales tax, and the contractor must not charge higher than their advertised government rate.
