About this file
This document set contains performance evaluation, reporting, and corrective action forms for the City of Phoenix's custodial services contract (RFP GGS-26-0368) for Fire Department facilities. Attachment A establishes the Custodial Service Performance evaluation framework, which assesses contractor compliance across four weighted categories: Quality of Service Delivery (60%), Management & Staffing (20%), Compliance of Services (10%), and Environmental Management (10%). Performance is scored on a scale with 90% and above considered passing, 89-90% requiring corrections, 84-88% needing improvement, 80-83% requiring immediate action, and 79% and below constituting failure requiring action. The evaluation includes specific metrics for contract performance, quality control, cleaning response time, staffing and supervision, cleaning schedules, daily communication, safety and security compliance, personnel training, energy conservation, and use of environmental products. Attachment B provides a daily reporting template for custodial contractor personnel to document work tasks, including employee names, dates, and start/lunch/leave times. Attachment C is a Corrective Action Form required for any scores of 90 and lower, which requires contractors to document immediate corrective actions taken, root cause analysis, detailed descriptions of how identified causes led to deficiencies, management actions implemented to prevent recurrence, implementation dates, management follow-up verification, and measures for sustaining corrective actions.
The contract for custodial services is a two-year agreement commencing August 1, 2027, with three one-year renewal options exercisable at the City's sole discretion. These forms establish the performance management framework that will govern contractor accountability throughout the contract term, with systematic evaluation and corrective action protocols to ensure service quality standards are maintained. The City of Phoenix utilizes its Procurement Portal for solicitation access, with technical support available through OpenGov and procurement inquiries directed to the listed procurement officer.
