About this file

This document is Attachment A and B from the Florida Department of Management Services, specifically a Mandatory Responsive Requirements and Contractor Experience Questionnaire form for a construction management (CM) solicitation. The form requires potential CM firms to provide comprehensive details about their business, including mandatory certifications that they are not on suspended vendor lists, have no delinquent state obligations, are registered with the Florida Department of State, and meet various legal and operational compliance standards. Respondents must confirm they have a licensed general or building contractor and have been prequalified by the department within the past 24 months.

The questionnaire demands extensive documentation about the firm's financial and operational history, including annual construction gross revenue for the past five years, current bonding capacity, years in business, full-time personnel breakdown, management team experience, and a detailed project history. Firms must disclose any failed projects, litigation history, and provide information on their ten most recently completed contracts, including project owners, locations, building types, construction budgets, and completion dates. The document requires transparency about project variations, potential liquidated damages, and ongoing construction projects, serving as a comprehensive vetting tool to assess a construction management firm's capability and reliability for potential state contracts.
