About this file
This document is a Past Performance Questionnaire (PPQ) for a Marine Corps University contract solicitation for 360-Degree Leadership Feedback Reviews. The solicitation (Request for Quote M00264-25-Q-0073) requires a contractor to provide a software tool for collecting, reviewing, analyzing, and reporting leadership feedback data, along with associated leadership training services. The PPQ is designed for references to evaluate the contractor's past performance across five key areas: customer satisfaction, timeliness, technical success, quality of performance, and cost control, using a rating scale from Unsatisfactory to Exceptional.
The document instructs offerors to distribute the PPQ to their past performance references, who must complete and email the questionnaire to specified Marine Corps contacts (genia.fouts@usmc.mil and jeffrey.a.sisk@usmc.mil) at least two days before the solicitation closes. The assessment aims to establish a comprehensive performance history for potential contractors, with detailed evaluation criteria covering technical execution, subcontract management, program management, personnel management, and cost control metrics. References are required to provide ratings and supporting comments for each performance category.
