About this file
This is a Request for Substitution/Exemption form issued by the City of Phoenix Public Transit Department's Compliance Section to manage changes to Disadvantaged Business Enterprise (DBE) utilization on public transit contracts. The form permits prime contractors to request approval for modifications to their proposed DBE participation, including the release or substitution of DBE subcontractors, changes to scope of work, or reductions to originally proposed amounts. Requests must be submitted to ptdcompliance@phoenix.gov prior to implementation, and no DBE substitutions, terminations, scope reductions, or amount changes are permitted without formal approval from the Compliance Section. The form requires prime contractors' authorized representatives to document their good faith efforts in sourcing DBE participation and provide supporting documentation justifying any requested exemptions or modifications.
Upon submission, the City's Contract Compliance Section reviews and approves or denies requests, with authority to revise the DBE goal and utilization percentages as appropriate. The form serves as an administrative control mechanism to ensure DBE program compliance and accountability throughout the contract term, requiring coordination between prime contractors and the Public Transit Department's compliance staff.
