About this file

This is a Labor Resume Form (Attachment Q) for a state and local contract opportunity issued by the State of Maryland for Third-Party Administrator Services for Flexible Spending Accounts. The form serves as a standardized template for bidders and offerors to document how proposed personnel meet minimum qualifications for specific labor categories. Bidders must complete one Labor Category Personnel Resume Summary for each proposed individual, clearly demonstrating how candidates satisfy the duties, responsibilities, and minimum qualifications outlined in the solicitation. The form requires candidates' names, contractor/company affiliations, education and training credentials, relevant work experience organized chronologically with specific dates, complete employment history, and professional references that the State may contact. The form emphasizes that candidates' relevant experience sections must be completed with substantive descriptions rather than references to external resumes, and candidates must provide specific employment dates when time-based requirements are specified.

The Labor Resume Form, effective July 2024, requires both contractor representatives and proposed individuals to sign and certify that the information provided is true and correct. Bidders must insert labor category titles and corresponding requirement descriptions from Appendix 3 of the solicitation into designated sections addressing education, experience, and duties. The form allows for supplemental documentation to support candidates' qualifications and establishes a uniform evaluation mechanism for assessing personnel proposed to fulfill contractual obligations. This standardized approach ensures consistent evaluation of proposed staff across all bidding organizations and facilitates the State's assessment of whether candidates possess the necessary qualifications to deliver Third-Party Administrator Services for Maryland's Healthcare Flexible Spending Account and Dependent Daycare Flexible Spending Account programs.

Attachment Q. Labor Resume Form

INSTRUCTIONS:

1. For each proposed person, complete one Labor Category Personnel Resume Summary to document how the proposed person meets each of the minimum requirements.

For example: If you propose John Smith, who is your subcontractor, and you believe he meets the requirements of the Group Facilitator, you will complete the top section of the form by entering John Smith’s name and the subcontractor’s company name. You will then complete the right side of the Group Facilitator form documenting how the individual meets each of the requirements. Where there is a time requirement such as three months’ experience, you must provide the dates from and to, showing an amount of time that equals or exceeds mandatory time requirement; in this case, three months.

2. Additional information may be attached to each Labor Category Personnel Resume Summary that may assist a full and complete understanding of the individual being proposed.
