Attachment # L-3 Past Performance Questionnaire For Official Use Only When completed, this document is Source Selection Information IAW FAR 2.101 and 3.104.

The 45th Contracting Squadron of the 45th Space Wing is in the process of competitively selecting a source for the Pad Safety Support Contract (PSSC) II. The scope of this acquisition will include operations and technical support required to augment and operationally implement the comprehensive safety program. The primary objective is to obtain superior, timely, responsible and affordable safety services in an environment of constantly changing mission requirements. The safety contractor must provide flexible customer support and visionary long-range planning while also providing dependable safety support that ensures public, launch base, and launch complex safety for all range users.

One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 10 Calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.

NOTE TO OFFERORS: This procurement could be similar to commercial supplies/services. Therefore, to assist the Government’s Past Performance Team in assessing your present and past performance on relevant commercial contracts, the following letter must be sent to your points of contact (POCs) for those commercial efforts that you identify to us in your past performance volume. Should you propose to use major subcontractors and/or joint venture members, you must obtain a similar client authorization letter from each entity. HOWEVER, it is your sole responsibility, as the offeror, to then send out these authorization letters with the Present/Past Performance Questionnaire to your own POCs and to those of your major subcontractors and/or joint venture members and include a copy in your past performance volume.

They are requesting that clients of companies who submit proposals in response to their RFP for the PSSC II effort be contacted, and that their participation in the validation process be requested. The offeror shall send out – and track the -- Present/Past Performance Questionnaire to each of the offeror’s, major subcontractors’, teaming contractors, and/or joint venture members’ Points of Contact (POCs). The responsibility to send out the Present/Past Performance Questionnaires rests solely with the offeror - i.e., it shall not be delegated to any subcontractors and/or joint venture partners. The completed Present/Past Performance Questionnaire shall be submitted directly to the Government not later than [Offeror to fill-in closing date]. Each of the offeror’s POCs may submit its completed Present/Past Performance Questionnaire either electronically, by fax or by mail. If sending electronically, the following applies:

NOTE TO OFFERORS: Past performance information concerning major subcontractors, teaming contractors, and/or joint venture members cannot be disclosed to a private party without the major subcontractor's or joint venture member’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing major subcontractor/joint venture partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the joint venture members/major subcontractors/teaming contractors identified in your proposal.
