GENERAL SUMMARY

Pond was retained by the United States Army Corps of Engineers (USACE) to provide an out-of-service (OOS) tank inspection for Tank 4 (Facility 5019) at Moody Air Force Base (AFB) in Valdosta, Georgia.

The OOS inspection was performed 20-21 May 2020. Results of this inspection identified recommended repairs/upgrades to fully comply with applicable sections of American Petroleum Institute (API) 650/653, Unified

Facilities Criteria (UFC) 3-460-01/3-460-03, Department of Defense (DoD) standard AW 78-24-27, National Fire

Protection Association (NFPA) 407, Occupational Safety and Health Administration (OSHA) guidelines, industry standards, and best management practices (BMP).

Tank 4 is a welded, carbon steel, vertical, aboveground Jet A (F-24) storage tank. The tank is assumed to be originally constructed to API 12C standards by an unknown manufacturer in 1960 and was repaired by Toltest in September

2006 to API 653, 3rd Edition, Addendum 1. The tank is 45’-0” in diameter and 34’-6” high with a nominal capacity of

10,000 Barrels (BBL) (420,000 gallons).

Testing and inspection activities were performed in accordance with (IAW) applicable sections of API 653, 5th

Edition. This report contains a detailed account of the tank inspection, non-destructive testing (NDT) methods, and conclusions derived from the inspection and testing activities. Findings from this inspection are further detailed in

Section B – Tank Inspection Comments. Mandatory and recommended repairs/upgrades identified in Section B are further classified and listed in Section C – Findings and Recommendations.

A. GENERAL PROJECT INFORMATION

1. API 653 OOS (INTERNAL) INSPECTION

The OOS inspection was performed 20-21 May 2020. Results of this inspection identified recommended repairs/upgrades to fully comply with applicable sections of API 650/653, UFC 3-460-01/3-460-03, DoD standard AW 78-24-27, NFPA 407, OSHA guidelines, and industry standards.

2. SITE INFORMATION

The Major Command at Moody AFB is the Air Combat Command. The 23rd wing is the home of the world famous “Flying Tigers”, combat search and rescue, and security forces warriors. The 23rd wing consists of “first in” combat search and rescue, close air support, forward air control, interdiction and force protection airmen who conduct worldwide operations in support of United States national security interests and the global war on terror. Additionally, under the wing’s command is the 563rd rescue group at Davis-Monthan AFB with an operating location at Nellis AFB.

B. TANK INSPECTION COMMENTS

1. TANK CONSTRUCTION

Tank 4 is a welded, carbon steel, vertical, aboveground Jet A (F-24) storage tank. The tank is assumed to have been constructed to API 12C standards by an unknown manufacturer in 1960 and was repaired by Toltest in September 2006 to API 653, 3rd Edition, Addendum 1. The tank is 45’-0” in diameter and

34’-6” high with a nominal capacity of 10,000 Barrels (BBL) (420,000 gallons).

2. TANK FOUNDATION

Tank 4 is provided a concrete ringwall foundation in good overall condition with no signs of spalling or cracking. Four (4) leak detection ports are provided between the new and original bottom. Liquid was not observed at the leak detection ports during inspection.

A settlement evaluation was performed during the inspection. The settlement evaluation was satisfied based on data collected. See Appendix A for settlement results.

3. TANK BOTTOM

The tank is provided a double bottom. The new tank bottom is cone-down with a center sump and constructed of lap-welded carbon steel plates with butt-welded annular carbon steel plates. The tank bottom was examined using magnetic flux leakage (MFL), ultrasonic thickness testing (UTT), and visual testing (VT) methods. Five (5) UTT measurements were taken on each bottom plate. UTT readings were also taken circumferentially every 2-feet within 3-inches of the shell-to-bottom weld on the annular plates. UTT measurements indicated no significant signs of metal loss on bottom or annular plates. VT was performed at bottom plate lap-welds, annular plate butt-welds, and the shell-to-bottom weld. No significant findings were observed. VT was limited due to internal coating.

An MFL bottom scan was performed on accessible areas of the tank bottom to detect top-side (TS) and soil-side (SS) metal loss. During the internal MFL scan of the tank bottom and annular plates, no significant TS or SS indications were identified.

Internal piping is provided with U-bolt pipe supports. Two (2) water draw-off U-bolt pipe supports are not provided isolation between the support and pipe. Minor contact point corrosion was observed at the time of inspection.

Two (2) internal bearing plates are within 2-inches of bottom plate lap welds. API mandates that bottom plate attachments are spaced a minimum 2-inches to avoid the heat affected zone and stress risers.

4. TANK SHELL

The tank shell is equipped with five (5) fully coated, butt-welded, carbon steel shell courses. Shell plates were examined using UTT and VT methods. Nine (9) UTT measurements were taken on each plate of the 1st shell course, three (3) UTT measurements on each insert plate, and ten (10) UTT measurements on each of the 2nd through 5th shell courses using the spiral staircase for access. UTT measurements indicated no significant signs of metal loss on shell plates.

Conduit provided for the Enraf ATG and Varec TLG is in direct contact with the tank chime and shell, hindering the ability to inspect 100% of the tank shell-to-bottom weld and accelerating coating failure.

Six (6) conduit supports are welded directly to the 1st and 2nd shell course horizontal seam weld. API mandates that shell attachments are spaced a minimum 3-inches to avoid the heat affected zone and stress risers.

An out-of-roundness (OOR) survey was conducted on the internal tank shell. The largest deviation from the tank nominal diameter was 1 1/8-inches at 3-feet above the shell-to-bottom weld. The maximum allowable deviation more than 1-foot above the shell-to-bottom weld per API 653 Table 10.2 is 3-inches. No significant signs of OOR were observed on the tank shell.

5. FIXED ROOF

The tank fixed roof was examined using UTT and VT methods. Five (5) UTT measurements were taken on each fixed roof plate. UTT measurements indicated no significant signs of metal loss on fixed roof plates.

The fixed roof is provided a center support beam constructed of two (2) I-beams for structural support of the fixed roof. The fixed roof center support beam is stitch welded, creating a crevice for fluid to collect, potentially accelerating coating failure and surface corrosion. Crevice corrosion and corrosion staining were observed during inspection.

Four (4) conduit supports provided for the Vito temperature/water probe are not sealed to the fixed roof. Minor coating failure was observed at the support-to-roof interface, potentially accelerating contact point corrosion.

6. TANK APPURTENANCES

6.1 TANK SHELL

Nozzles and appurtenances associated with the shell were inspected using UTT and VT methods. Four (4) UTT measurements were taken on each nozzle with a welded connection greater than 1-inch at four (4) quadrants (0°, 90°, 180°, and 270°). One (1) UTT measurement was taken on each associated reinforcement pad or thickened insert plate. UTT measurements indicated no significant signs of metal loss on the nozzles, reinforcing pads, or thickened insert plates.

Tank 4 is equipped with fourteen (14) shell nozzle penetrations: two (2) 36-inch manways, one (1) 8-inch receipt, two (2) 1 ½-inch LLA/LLLA nozzle penetrations, two (2) 1 ½-inch

HLA/HHLA penetrations, two (2) 2-inch HLCV float pilot penetrations, one (1) 1 ½-inch plug, one (1) 16-inch main suction, one (1) 4-inch low suction, one (1) 1 ½-inch water draw-off, and one (1) 6-inch aqueous film forming foam (AFFF) discharge chamber penetration.

External coating on the LLA chamber assembly exhibits flaking and base metal exposure with surface corrosion. Manway 1 and Manway 2 are provided hinges rather than davit arms with grease fittings and are not IAW DoD standard design. Existing hinges are welded directly to the nozzle flange and are not attached to the tank. Confined space labeling on each manway is faded and peeling.

6.2 INTERNAL FLOATING ROOF

Tank 4 is provided an aluminum honeycomb floating pan with a double wiper seal. The internal floating roof (IFR) is in good overall condition with no signs of fluid accumulation on top of the pan. A new double wiper seal system and all IFR penetration vapor seals are to be installed before the tank is returned to service.

6.3 FIXED ROOF

Nozzles and appurtenances associated with the fixed roof were inspected using UTT and VT methods. Four (4) UTT measurements were taken on each nozzle with a welded connection greater than 1-inch at four (4) quadrants (0°, 90°, 180°, and 270°). One (1) UTT measurement was taken on each associated reinforcement pad. UTT measurements indicated no significant signs of metal loss on nozzles or reinforcing pads.

The fixed roof is provided fifteen (15) nozzle penetrations: four (4) 1-inch threaded caps, one

(1) 1-inch threaded plug, one (1) 2-inch Varec TLG, two (2) 2-inch threaded plugs, one (1) 6-inch Vito temperature/water probe, one (1) 6-inch blind flange, one (1) 36-inch by 36-inch

IFR inspection hatch equipped with a 6-inch manual gauging hatch and IFR access ladder, one

(1) 10-inch Enraf ATG, one (1) 72-inch by 24-inch IFR installation hatch, and two (2) 1-inch

IFR grounding cables.

External coating on the Enraf ATG, Vito temperature/water probe, two (2) anti-rotation cables, one (1) IFR inspection hatch, one (1) blind flange, three (3) threaded plugs, and two

(2) threaded caps nozzles exhibit flaking and base metal exposure with surface corrosion.

The IFR inspection hatch is provided with one (1) 6-inch manual gauging hatch. Currently, there is no means of locking the manual gauging hatch cover while not in use. Confined space labeling on the IFR inspection hatch is faded and peeling.

6.4 GAUGING

The tank is provided a manual gauging hatch for manual gauging operations. An Enraf ATG and Vito temperature/water probe are provided for automatic tank gauging and sampling operations. A mechanical Varec TLG is provided for monitoring product level from the ground and is attached to the IFR. The Varec TLG failed the head field test, was inconsistent with the

Enraf ATG, and has two (2) conduit support brackets missing. The stilling well provided for the Vito temperature/water probe is not fully slotted and may provide inaccurate readings during filling and issuing operations.

6.5 LEVEL ALARMS

The tank is provided with independent, shell mounted alarms in the low, low-low, high, and high-high positions as well as a HLCV float pilot for overfill protection. According to facility personnel, all level alarms and the HLCV float pilot are functioning properly. Piping and nozzle connections for the HLA, HHLA, and HLCV float pilot are provided with threaded connections. Threaded connections are no longer used as they are a potential leak path.

The tank is not provided with a standard product saver tank (PST) with means of testing the level alarms and HLCV system. The PST system should include tubing to the HLA, HHLA, and

HLCV float pilot to facilitate wet testing of the chambers.

6.6 STAIRS AND PLATFORMS

The tank is provided a spiral staircase with one (1) upper platform for fixed roof access. The

HLCV float pilot extends into the path of the spiral staircase, allowing 20-inches clearance between the appurtenance and staircase railing. OSHA mandates a minimum 26-inches clearance between appurtenances and staircase railings. Handrail heights for the spiral staircase are 37-inches tall, where API requires handrails to be between 30-inches and 34-inches high.

External coating for handrails and balusters provided for the secondary containment access stairs and one (1) main suction crossover stairs exhibit failure with flaking, base metal exposure, and surface corrosion. No sealant is provided for the access staircase, allowing for fluid to escape in the event of a spill.

6.7 GROUNDING AND BONDING

The tank shell is equipped with four (4) grounding cables. Three (3) of the four (4) grounding cables exhibit damage, disbondment, and fraying, potentially decreasing the efficiency of the grounding system.

7. TANK AND PIPING COATING

The tank shell, fixed roof, and piping are provided with an epoxy polyurethane coating system. For TS corrosion protection, the tank bottom is provided with a three-coat epoxy coating system. Internal coating for the tank bottom and attachments exhibit failure with base metal exposure, surface corrosion, and corrosion staining. External coating for the tank shell and fixed roof exhibits widespread chalking, indicating the coating system is near the end of its life cycle. External coating on the tank chime, receipt, receipt thermal relief crossover piping, PST, and secondary containment drain piping exhibits flaking, cracking, and base metal exposure with surface corrosion.

8. TANK PIPING SYSTEM

Piping within the containment dike is constructed of carbon steel. Ball Valve #385, upstream of the 8-inch HLCV, was weeping at the flange connection during inspection. UFC requires that valves located immediately upstream and downstream of the HLCV be double block and bleed (DB&B) valves for positive isolation of system components during maintenance operations. The low suction nozzle is not provided with a main suction branch connection, limiting the tank low suction system to aid in fuel removal during maintenance and inspection activities.

The main suction is provided with adjustable saddle supports and a U-bolt pipe support. One (1) main suction U-bolt pipe support, adjacent to the tank shell, is not anchored to the secondary containment dike floor and may not be adequately supporting the main suction pipe. One (1) receipt adjustable saddle pipe support has a misaligned isolation pad and three (3) receipt thermal relief crossover piping are not provided with Teflon isolation, or equivalent. Contact point corrosion was observed during inspection.

The receipt diffuser is misaligned at the flange connection and is in direct contact with the inner most pipe support leg, potentially adding additional unintended stresses to the support. Contact point corrosion was observed during inspection. Additionally, the receipt diffuser orientation is such that the flow of fuel into the tank encounters appurtenances less than 180° from the diffuser. AW standard design mandates that the receipt diffuser is oriented such that the flow of fuel into the tank does not encounter appurtenances for at least 180°.

PST piping is provided threaded connections, one (1) sight flow indicator, and no isolation between pipe supports. Threaded connections and sight flow indicators are no longer used in this application as they are a potential leak path. The PST piping is provided an inadequate pipe supporting system.

Current configuration is using the receipt piping for support with no isolation between the pipe support and receipt/PST piping. Coating failure and surface corrosion was observed during inspection. Flow arrows for the receipt piping exhibit fading and peeling.

9. FIRE PROTECTION SYSTEM

For internal fire protection, an aluminum honeycomb IFR and double wiper seal is provided. Prior to the installation of the IFR, an AFFF system was provided. Currently, the OOS AFFF system is still attached to the tank.

10. SECONDARY CONTAINMENT

Tank 4 is located within a concrete containment dike lined with a flexible membrane liner (FML). The containment dike is capable of holding approximately 518,000 gallons of liquid, satisfying the 1-foot

UFC secondary containment capacity requirement. The secondary containment is in good overall condition with no visual signs of pathways for liquid to escape in the event of a spill.

The drain provided for the secondary containment is equipped with a manually operated valve and was tested during inspection. The drain was filled with water and let sit for 24-hours to test for leaks.

After 24-hours of testing, the valve did not leak, resulting in a sealed draining system.

11. CATHODIC PROTECTION

Tank 4 is equipped with an ICCP system to protect the tank bottom from SS corrosion. The rectifier for

Tank 4 was operating at 23.31 volts and 0.24 amps. Three (3) potential readings were taken for Tank

4. All readings met the -850 millivolt National International (NACE) CP acceptance criteria. CP readings are consistent with the previous CP report dated May 2018.

C. FINDINGS AND RECOMMENDATIONS

Based on the OOS API 653 inspection of Tank 4 located at Moody AFB in Valdosta, Georgia mandatory and recommended repair/upgrades have been identified. The following recommendations outline observations from this inspection. All repair recommendations are based on applicable sections of API 650/653, UFC 3-

460-01/3-460-03, DoD Standard AW 78-24-27, NFPA 407, and industry standards.

1. MANDATORY REPAIRS PER API 650/653, UFC 3-460-01/3-460-03, OR DoD AW 78-24-27

Mandatory repairs represent items that require immediate attention, in order to prevent imminent risk to system operators, equipment integrity, or the adjacent environment. The tank should remain out of service until these repairs are completed.

There are no mandatory repairs requiring Tank 4 to remain out-of-service.

2. SHORT-TERM RECOMMENDED REPAIRS

Short-term repairs relate to the long-term preservation of the asset. Short-term repairs represent items that should be addressed in a timely manner in order to prevent future potential risks to system operators, equipment integrity, or the adjacent environment. These items may be required by federal, state, and local codes or by military criteria.

2.1 Weeping Receipt Ball Valve

OBSERVATION: Ball Valve #385, positioned upstream of the 8-inch HLCV, was weeping at the flange connection during inspection and is a ball valve.

REFERENCE: UFC 3-460-03, Section 6-1.5

RECOMMENDATION: Replace gasket at Valve #385 flange connection to prevent future weeping.

Additionally, replace both ball valves on the receipt piping upstream of the HLCV with two (2) 8-inch DB&B valves.

2.2 Tank Bottom Internal Coating

OBSERVATION: Internal coating for the tank bottom exhibits spot failure with base metal exposure, surface corrosion, and corrosion staining.

REFERENCE: UFC 3-460-01, Section 8-3.9 (A)

RECOMMENDATION: Provide approximately 190 ft2 (10%) of spot coating repair to the internal tank bottom.

2.3 Fixed Roof Center Support

OBSERVATION: The fixed roof center support is stitch welded. Corrosion staining were observed during inspection.

REFERENCE: UFC 3-460-01, Section 8-3.9 (a)

RECOMMENDATION: Provide sealant at the fixed roof center support crevices, approximately 100 linear feet.

2.4 Tank Shell External Coating

OBSERVATION: External coating for the tank shell exhibits widespread chalking indicating the coating system is near the end of its life cycle.

REFERENCE: UFC 3-460-01, Section 8-3.10 (A)

RECOMMENDATION: Provide a full recoat of the tank shell, approximately 5,900 ft2 (100%). Prior to recoat, perform a coating condition survey, including an adhesion test, to determine if a full recoat is required or if a topcoat reapplication is sufficient.

2.5 Tank Chime External Coating

OBSERVATION: External coating on the tank chime exhibits flaking and base metal exposure with surface corrosion.

REFERENCE: UFC 3-460-01, Section 8-3.10 (A)

RECOMMENDATION: Provide approximately 30 ft2 of spot coating repair on the tank chime.

2.6 Tank Fixed Roof External Coating

OBSERVATION: External coating for the tank fixed roof exhibits widespread chalking with minor surface corrosion. Chalking indicates the coating system is near the end of its life cycle.

REFERENCE: UFC 3-460-01, Section 8-3.10 (A)

RECOMMENDATION: Provide a full recoat of the tank fixed roof, approximately 2,000 ft2 (100%). Prior to recoat, perform a coating condition survey, including an adhesion test, to determine if a full recoat is required or if a topcoat reapplication is sufficient.

2.7 Receipt and PST Piping External Coating

OBSERVATION: External coating on the receipt, receipt thermal relief crossover, PST, and secondary containment drain valve piping exhibits flaking, cracking, and base metal exposure with surface corrosion.

REFERENCE: UFC 3-460-01, Section 9-10

RECOMMENDATION: Provide approximately 1,600 ft2 of external coating repair on the receipt, receipt thermal relief crossover, PST, and drain valve piping.

2.8 Tank Nozzle External Coating

OBSERVATION: External coating on the following nozzles exhibit failure with base metal exposure with surface corrosion:

• LLAs – 10 ft2

• Enraf ATG - 10 ft2

• Vito temperature/water probe - 5 ft2

• Two (2) anti-rotation cables - 5 ft2

• One (1) IFR inspection hatch - 5 ft2

• One (1) blind flange - 5 ft2

• Three (3) threaded plugs - 5 ft2

• Two (2) threaded caps - 5 ft2

REFERENCE: UFC 3-460-01, Section 8-3.10 (A)

RECOMMENDATION: Provide approximately 50 ft2 of spot coating repair on the tank nozzles.

2.9 Access and Crossover Stairs External Coating

OBSERVATION: External coating for handrails and balusters for the access and crossover stairs exhibit failure with flaking, base metal exposure, and surface corrosion.

REFERENCE: UFC 3-460-03, Appendix F, Item 6

RECOMMENDATION: Provide approximately 40 ft2 of external coating repair for the access and crossover stairs.

2.10 Access Stair Sealant

OBSERVATION: No sealant is provided for the access staircase, allowing for fluid to escape in the event of a spill.

REFERENCE: UFC 3-460-03, Section 8-10.2.1

RECOMMENDATION: Provide 20 linear feet of flexible, fuel resistant, UV-resistant sealant to one (1) access staircase.

2.11 PST Piping

OBSERVATION: PST piping is provided threaded connections, no isolation between pipe supports, and an inadequate pipe supporting system.

REFERENCE: UFC 3-460-01, Section 9-8

RECOMMENDATION: Provide flanged or socket welded connections, remove one (1) sight flow indicator, and provide the piping system with pipe supports IAW UFC requirements.

2.12 PST Piping

OBSERVATION: PST piping is provided one (1) sight flow indicator.

REFERENCE: AW 78-24-27, Sheet D.13, Detail 5

RECOMMENDATION: Remove one (1) sight flow indicator on PST piping. Sight flow indicators are no longer used as they are a potential leak path.

3. LONG-TERM RECOMMENDED REPAIRS

Long-term repairs that relate to repair/upgrade of an asset to bring it up to the current standard from the standard at time of construction. Long-term repairs represent items that should be addressed in a timely manner in order to prevent future potential risks to system operators, equipment integrity, or the adjacent environment.

3.1 Low Suction Nozzle

OBSERVATION: The low suction nozzle is not provided with a main suction branch connection.

REFERENCE: UFC 3-460-01, Table 8.1 (Z)

RECOMMENDATION: Provide a branch connection between the low suction nozzle and main suction pipeline IAW AW DoD standard design.

3.2 Main Suction Pipe Support

OBSERVATION: One (1) main suction U-bolt pipe support, adjacent to the tank shell, is not anchored to the secondary containment dike floor.

REFERENCE: UFC 3-460-03, Section 6-1.5 (D), AW 78-24-33, Sheet CD.11

RECOMMENDATION: Anchor one (1) main suction pipe support to the secondary containment dike floor.

3.3 Internal Receipt Diffuser

OBSERVATION: The receipt diffuser is misaligned at the flange connection and is in direct contact with the inner most pipe support leg, potentially adding additional unintended stresses to the support. Contact point corrosion was observed during inspection.

Additionally, the receipt diffuser orientation is such that the flow of fuel into the tank encounters appurtenances less than 180°.

REFERENCE: UFC 3-460-03, Section 6-1.5 (A); AW 78-24-27, Sheet D-08, Detail 1, Note 2

RECOMMENDATION: Rework the receipt diffuser and associated pipe support to allow for proper diffuser movement and provide adequately support. Additionally, orientate the receipt diffuser such that the flow of fuel into the tank does not encounter appurtenances for at least 180°. Consider flipping the receipt nozzle at the flange connection to meet the 180° criteria.

3.4 Bottom Plate Weld Spacing

OBSERVATION: Two (2) internal bearing plates are within 2-inches of bottom plate lap welds.

REFERENCE: API 650, Figure 9.13

RECOMMENDATION: Perform magnetic particle (MT) or dye penetrant (PT) testing IAW API 650 at two

(2) bearing plates within 2-inches of bottom plate lap welds.

3.5 Shell Plate Weld Spacing

OBSERVATION: Six (6) conduit support brackets are welded directly to the 1st and 2nd shell course seam weld and one (1) stair tread is welded within 3-inches of the shell-to-bottom-weld.

REFERENCE: API 650, Section 5.7.3

RECOMMENDATION: Perform MT or PT testing IAW API 650 at six (6) conduit support brackets welded directly to the 1st and 2nd shell course seam weld and one (1) stair tread welded within 3-inches of the shell-to-bottom-weld..

3.6 Receipt and Thermal Relief Crossover Piping

OBSERVATION: Three (3) PST pipe supports and one (1) receipt adjustable saddle pipe support are not provided with Teflon isolation, or equivalent, or the isolation pad has shifted.

REFERENCE: UFC 3-460-03, Section 6-1.5, AW 78-24-33, Sheet CD.11

RECOMMENDATION: Provide three (3) PST pipe supports and one (1) receipt pipe support with Teflon isolation, or equivalent.

3.7 Manual Gauging Hatch

OBSERVATION: The cover for the manual gauging hatch is not provided a latch.

REFERENCE: BMP

RECOMMENDATION: Provide a latch for the manual gauging hatch to secure the cover while not in use.

3.8 Fixed Roof Conduit Support

OBSERVATION: Four (4) conduit supports for the Vito temperature/water probe are not sealed to the fixed roof.

REFERENCE: UFC 3-460-01, Section 8-11

RECOMMENDATION: Fully seal four (4) conduit supports for the Vito temperature/water probe to the fixed roof. Coating failure was observed at the time of inspection.

3.9 Vito Temperature/Water Probe Stilling Well

OBSERVATION: The stilling well provided for the Vito temperature/water probe is not fully slotted.

REFERENCE: UFC 3-460-01,Table 8-1 (W)

RECOMMENDATION: Fully slot the stilling well provided for the Vito temperature/water probe.

3.10 AFFF System

OBSERVATION: The tank is provided an AFFF and an IFR for fire prevention.

REFERENCE: UFC 3-460-01, Section 2-13.4.2

RECOMMENDATION: Remove the AFFF system, to include the discharge chamber, as it is no longer in use. Before AFFF removal, consult the local Fire Chief for approval. If approval is not granted, coat piping red IAW MIL-STD-161H.

3.11 Grounding Cable

OBSERVATION: Three (3) of the four (4) grounding cables exhibit damage, disbondment, and fraying.

REFERENCE: API 653, Section 6.3.2.3

RECOMMENDATION: Replace or repair three (3) grounding cables provided for Tank 4.

3.12 HLA, HHLA, and HLCV Float Pilot Piping

OBSERVATION: Piping and nozzle connections for the HLA, HHLA, and HLCV float pilot are provided with threaded connections.

REFERENCE: UFC 3-460-01, Section 8-11 and 9-8 (E)

RECOMMENDATION: Provide the HLA, HHLA, and HLCV float pilot with flanged or socket welded connections.

3.13 PST System

OBSERVATION: The tank is not provided a standard PST with means of testing the level alarms and

HLCV float pilot.

REFERENCE: AW 78-24-27, Sheet D.13 Detail 5

RECOMMENDATION: Provide the tank with a PST IAW AW standard design. The PST system should include tubing to the HLA, HHLA, and HLCV float pilot to facilitate wet testing of the chambers.

3.14 Varec TLG

OBSERVATION: The Varec TLG failed the head field test, was inconsistent with the Enraf ATG, and has two (2) conduit support brackets missing.

REFERENCE: UFC 3-460-01, Table 8-1 (Q)

RECOMMENDATION: Repair the Varec TLG system and provide two (2) Varec TLG brackets with U-bolt supports. Per facility request, remove the Varec TLG system.

3.15 Manway 1 and Manway 2 Davit Arms

OBSERVATION: Manway 1 and Manway 2 are not equipped with davit arms provided with grease fittings and are welded directly to the manway flange.

REFERENCE: API 650, Section 5.8.3.5; AW 78-24-27, Sheet D.10

RECOMMENDATION: Provide Manway 1 and Manway 2 davit arms with grease fittings.

3.16 Internal Pipe Supports

OBSERVATION: The internal pipe supports for two (2) water draw-off supports are not provided with Teflon isolation, or equivalent. Minor contact point corrosion was observed.

REFERENCE: AW 78-24-27, Sheet D.08 Detail 6

RECOMMENDATION: Provide Teflon isolation or Teflon coated u-bolts at two (2) water-draw supports.

3.17 HLCV Float Pilot Clearance

OBSERVATION: Clearance between the HLCV float pilot and spiral staircase handrail is approximately 20-inches, less than OSHA required 26-inch clearance. Additionally, handrails provided for the spiral staircase are 37-inched height where API mandates handrail heights be between 30-inches and 34-inches.

REFERENCE: OSHA 1910.25 (d)(1); API 650, Table 5.18 (6)

RECOMMENDATION: Extend the stair treads or provide a work platform at the HLCV float pilot to allow for minimum 26-inches of clear space between the spiral staircase handrail and appurtenance. Additionally, lower the handrails provided for the spiral staircase to have a maximum height between 30-inches and 34-inches.

3.18 Confined Space Labeling

OBSERVATION: Confined space labeling on the IFR inspection hatch and two (2) manways exhibit fading and peeling.

REFERENCE: API 650, Section 5.7.2.10

RECOMMENDATION: Replace confined space labeling for the IFR inspection hatch and two (2) manways.

3.19 Enraf ATG and Varec TLG Conduit

OBSERVATION: Conduit provided for the Enraf ATG and Varec TLG is in direct contact with the tank chime and shell. No contact point corrosion was observed.

REFERENCE: BMP

RECOMMENDATION: Rework the conduit provided for the Enraf ATG and Vito temperature/water probe to avoid direct contact with the tank chime and shell.

3.20 Receipt Piping Flow Arrows

OBSERVATION: Directional flow arrows for the receipt piping exhibit fading and peeling.

RECOMMENDATION: Replace directional flow arrows for the receipt piping.
