About this file
This is a Past Performance Questionnaire template designed for the Library of Congress to collect references on contractor performance for evaluation purposes in source selection.
The questionnaire is structured in three sections to be completed by different parties. Section I requires the offeror to provide basic contract information including contractor name, UEI, contract name/program, contract number, type, period of performance, duration, amounts, and customer point of contact details. Sections II and III are completed by client references and must be submitted directly to the Library of Congress Contracting Officer Cristina Vega (cvega@loc.gov) and Contract Specialist Kevin Barnes (kbarnes@loc.gov), not returned to the offeror. References evaluate contractor performance across five rating categories—Prime Contractor's Management Practices, Prime Contractor's Performance, Prime Contractor's Delivery Schedules, Summary assessment, and Additional Comments—using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Specific evaluation criteria include management structure and responsiveness, reporting effectiveness, communication quality, value of performance reports, staffing adequacy, staff skill levels, quality control, contract transition, deadline compliance, and pricing performance. References must verify accuracy of information provided by the offeror and certify the accuracy of their responses by signature. Responses are treated as source selection sensitive information. Handwritten responses are acceptable, and references may attach additional pages as needed.
