About this file

This is a Past Performance Questionnaire template for evaluators to complete regarding contractor performance under the R402 Real Estate Acquisition Support Services (FY21 New Broker IDIQ) contract opportunity (Solicitation 36C10F26R0045). Completed questionnaires must be submitted via email to Kevin Armillotti, Danny Killian, and Fritz Fabien at the VA's Department of Veterans Affairs Office of Construction and Facilities Management by a specified deadline to be inserted by the Government.

The questionnaire requests evaluator information and details about the referenced contractor's company, contract, and services performed. Evaluators must rate the contractor's performance on twelve criteria using a five-point scale (1=Unsatisfactory through 5=Outstanding) or mark responses as Not Applicable. Performance categories assessed include schedule adherence and milestone achievement, personnel qualifications and staffing continuity, communication effectiveness, quality control processes, issue identification and problem-solving, responsiveness to changing conditions and customer direction, support for customer-led negotiations, correction of deficiencies, timely resolution of issues, and professional and ethical conduct. The questionnaire also requires disclosure of any written performance notices, such as cure notices, corrective action requests, or termination notices. For ratings of Outstanding, Marginal, or Unsatisfactory, brief factual explanations must be provided in the Comments/Remarks fields, with space for additional comments regarding the contractor's overall performance.
