About this file

This is a Subcontractor Checklist template for use in responding to Solicitation No. 693JJ926R000067 for Countermeasures That Work (CMTW) updates. The checklist is a required appendix that offerors must complete for each proposed subcontractor or consultant before NHTSA will provide consent to any subcontract arrangement.

The checklist requires offerors to document specific information for each named subcontractor/consultant, including the subcontractor's name and address, the position or portion of work to be performed, proposed subcontract type and value, and the basis for determining price reasonableness in accordance with FAR 15.404-3. Additionally, offerors must indicate whether adequate price competition was obtained, whether the subcontractor is excluded in SAM (System for Award Management), and must provide confirmations regarding attached documentation including letters of commitment, negotiation memorandums, and cost/price analysis reports. The checklist requires signature and date certification from the prime offeror. This is a template document and does not address specific deliverables or service requirements related to the underlying solicitation.
