About this file
This is a Past Performance Questionnaire template used for evaluating contractor performance on the NHTSA Traffic Records Program contract (RFP 693JJ926R000030). The document serves as a standardized form for government evaluators to assess and document a contractor's past performance during the proposal evaluation process.
The questionnaire contains two primary components: a Performance Rating Form and a Supplemental Comment Form. The Performance Rating Form evaluates contractors across nine key performance elements using a four-point adjectival scale (Outstanding, Satisfactory, Unsatisfactory, or Other/N/A). The evaluated criteria include quality of technical approach, effective and efficient use of resources, effective and efficient use of subcontractors, quality of performance and customer satisfaction, business behavior, communication capabilities, cost control, timeliness of performance, and understanding of requirements. Additionally, the form includes six yes/no questions (items 10-15, criteria not specified in the provided template). The Supplemental Comment Form allows evaluators to provide detailed narrative feedback on any of the performance elements. Government representatives conducting these evaluations must inform references that individual names will not be disclosed to the contractor. The form is designed to be completed by an evaluator from the contractor's past customer agency, documenting their direct experience with the contractor's performance on similar work.
