About this file
This is a Past Performance Evaluation Questionnaire template for solicitation 75F40126R00051. The form is designed to be completed in two parts: Section 1 by the offeror providing information about themselves and a referenced past contract, and Section 2 by the client agency or company providing their evaluation of the contractor's performance.
Section 1 requires the offeror to provide contract identification details including offeror name, address, and point of contact information; reference information for the agency or company to whom the questionnaire was submitted; location and description of work; relevance of the referenced contract to current requirements; problems encountered and corrective actions; company capacity (prime, subcontractor, or key personnel); and contract information including contract number, type, period of performance, original and final contract amounts, and completion dates. Section 2, completed by the client, requests a description of work performed and the contractor's role, reasons for any differences between original and final contract amounts or completion dates, and detailed performance ratings across seven evaluation categories: Timeliness of Performance (adherence to delivery schedules and timely submission of reports); Quality of Work (accuracy and completeness of documentation, problem-solving ability, and conformance to specifications); Business Relations (adequate resource commitment, timely submission of change orders, responsiveness to technical direction, and staff interfacing); Cost Control (accurate pricing information for change orders and warranted change order requests); Management of Key Personnel (appropriate skills and personnel retention); and General satisfaction and overall rating. Each rating category offers five options (Outstanding, Above Average, Satisfactory, Marginal, Unsatisfactory) with required supporting comments. The form concludes with evaluator information fields and signature block.
