About this file
This is a Past Performance Questionnaire template for federal contract opportunity 030ADV25R0045 related to the Library of Congress Madison Rooms Audio and Visual Equipment project. The questionnaire is designed to evaluate contractor performance using a five-point rating scale ranging from Outstanding to Unsatisfactory, with specific definitions for each rating level that assess whether performance meets contractual requirements and the effectiveness of corrective actions taken.
The evaluation form contains ten performance assessment questions covering overall contract management, quality of products and services, project management effectiveness, schedule compliance, reporting timeliness, subcontractor management, personnel competency and communication, cost performance, and contract modifications. The questionnaire must be completed by previous customers of the proposing contractor and returned via email to Ramon Samuel at rsamuel@loc.gov with copy to Moji Adejuwon at moad@loc.gov by August 25, 2025 at 12:00pm ET. The form requires general contract information including company details, contract number, type, performance period, dollar value, and description, along with narrative explanations for any Marginal or Unsatisfactory ratings. This past performance evaluation is specifically for contractors proposing to remove old equipment and deliver/install new audio, video, and lectern equipment for the Library of Congress.
