About this file
This is a Past Performance Report Form used to evaluate contractor performance on previous contracts relevant to solicitation N0018926RL023. The form must be completed and returned to NAVSUP Fleet Logistics Center Norfolk by the due date of solicitation. Responses should be submitted to three contracting officers: Kendall Mulherin (kendall.l.mulherin.civ@us.navy.mil, 771-229-3568), Benjamin Fisher (benjamin.d.fisher8.civ@us.navy.mil, 771-229-0828), and William Spencer (william.p.spencer.civ@us.navy.mil, 771-229-3528). The form requests information about a contractor's previous contract performance, including contract number, amount, type, period of performance, and business address, along with contact information for both the company being evaluated and the organization completing the questionnaire.
The questionnaire evaluates past performance across six quality dimensions: recruiting capability for personnel with required skills, certifications, experience, and security clearances; workforce retention and stability; quality and compliance with contract requirements, including customer satisfaction, rework instances, and innovative applications; timeliness of performance schedules and submission of reports and invoices; contractor responsiveness to customer concerns and problem resolution, including cost control and adherence to contract pricing; and additional observations regarding contractor strengths, weaknesses, overall satisfaction with the award decision, awareness of similar contracted efforts, and identification of other relevant points of contact. This form is designed to support source selection and evaluation for a Firm-Fixed Price IDIQ contract for Naval Information Warfighting Development Center Electronic Warfare Data support services.
