About this file

This is a certification form attachment required by the Florida Department of Management Services for the Natural Gas Supply ITB No. 25-83101601-ITB-V2 solicitation, mandated under Florida Statutes section 287.1346(4)(b). The document requires a member of a bidding company's senior management—defined as chief executive officers, assistant chief executive officers, chief financial officers, chief personnel officers, or employees performing similar functions—to certify that the commodities being offered have not been produced, in whole or in part, by forced labor. The form requires the entity name, senior management member's printed name and title, signature, and date of certification. Completion of this certification is mandatory when a company submits a response to a solicitation for the provision of commodities and before entering into or renewing a contract for such provisions.

The certification serves as a compliance mechanism for Florida's procurement regulations regarding forced labor standards in commodity procurement. Bidders must complete and submit this attachment as part of their overall bid response to be considered responsive and responsible. The form does not contain pricing information, technical specifications, or other substantive contract terms, but rather functions as a required attestation document that demonstrates the bidder's commitment to ethical sourcing practices and compliance with Florida state law.
