About this file

This is a certification attachment document issued by the Florida Department of Management Services for the Natural Gas Supply ITB No. 25-83101601-ITB-V2, a statewide procurement for natural gas supply. The ITB seeks to establish a State Term Contract with potential for multiple awards across regions or statewide, as determined in the State's best interest. Interested vendors must submit responses through the MyFloridaMarketPlace Sourcing Event via the SAP Business Network and are responsible for monitoring the Vendor Information Portal (VIP) throughout the procurement process. The procurement is managed by Alyssa Wallace, Purchasing Analyst, Florida Department of Management Services, located at 4050 Esplanade Way, Suite 360.6Y, Tallahassee, FL 32399-0950, with contact information at (850) 410-2426 and Alyssa.Wallace@dms.fl.gov.

This attachment specifically addresses the Provision of Commodities Produced by Forced Labor requirement under Florida Statutes section 287.1346(4)(b). As a mandatory provision, a member of the bidding company's senior management—defined to include chief executive officers, assistant chief executive officers, chief financial officers, chief personnel officers, or employees performing similar functions—must complete and certify that the commodities being offered to the Department have not been produced, in whole or in part, by forced labor. The certification requires the senior management member's printed name, title, signature, and date, and must be submitted with the bid response prior to contract execution or renewal.
